Description
Overview of the medicine
Tygacil (Tigecycline) is an intravenous antibiotic used to treat serious bacterial infections, including complicated skin and skin structure infections (cSSSI), complicated intra-abdominal infections (cIAI), and community-acquired bacterial pneumonia (CABP). It belongs to the glycylcycline class of antibiotics.
Uses & Indications
Dosage
Adults
Initial dose of 100 mg, followed by 50 mg every 12 hours for 5 to 14 days.
Elderly
No dosage adjustment required based on age.
Renal_impairment
No dosage adjustment required.
How to Take
Administered intravenously (IV) over 30 to 60 minutes.
Mechanism of Action
Tigecycline inhibits protein synthesis in bacteria by binding to the 30S ribosomal subunit, thereby preventing the entry of aminoacyl-tRNA into the A-site of the ribosome. This action is primarily bacteriostatic.
Pharmacokinetics
Onset
Rapid, within minutes of infusion.
Excretion
Primarily excreted unchanged via bile/feces (59%), with a smaller portion via urine (33%).
Half life
Approximately 42 hours (terminal half-life).
Absorption
Administered intravenously, thus 100% bioavailability.
Metabolism
Not extensively metabolized. Glucuronidation is the primary metabolic pathway.
Side Effects
Contraindications

Drug Interactions
Warfarin
Tigecycline may prolong prothrombin time (INR).
Oral Contraceptives
May reduce efficacy of oral contraceptives.
Storage
Store unopened vials at 20-25°C (68-77°F), excursions permitted to 15-30°C (59-86°F). Reconstituted solution should be used immediately.

Clinical Trials
Extensive clinical trials have demonstrated efficacy in complicated skin and skin structure infections, complicated intra-abdominal infections, and community-acquired bacterial pneumonia. Pooled analyses of trials showed an increased all-cause mortality risk, leading to a black box warning.
Lab Monitoring
- Liver function tests (LFTs)
- Renal function tests
- Blood counts (CBC)
- INR (if on warfarin)
Doctor Notes
- Due to the black box warning on increased mortality, tigecycline should be reserved for infections where alternative treatments are not appropriate.
- Careful monitoring for adverse events, especially gastrointestinal and liver function, is essential.
- Appropriate cultures should be obtained prior to therapy to guide treatment.
Patient Guidelines
- Complete the full course of treatment even if you feel better.
- Report any severe or persistent side effects to your doctor.
- Inform your doctor about all other medications you are taking.
- Do not use for infections other than those prescribed.
Missed Dose Advice
If a dose is missed, administer it as soon as possible. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
Tigecycline may cause dizziness. Patients should be cautious when driving or operating machinery.
Lifestyle Advice
- Maintain good hydration.
- Avoid alcohol during treatment.
- Practice good hygiene to prevent further infections.
