Description
Overview of the medicine
Tysul 50 mg Tablet contains Sulpiride, an atypical antipsychotic primarily used to treat schizophrenia, vertigo, and certain gastrointestinal disorders like irritable bowel syndrome and dyspepsia.
Uses & Indications
Dosage
Adults
For vertigo/GI disorders: 50 mg 2-3 times daily. For psychosis: 200-400 mg twice daily, up to 1600 mg daily in divided doses. Dosage should be individualized.
Elderly
Lower starting doses (e.g., 50-100 mg daily), with gradual titration. Monitor closely for side effects due to increased sensitivity.
Renal_impairment
Dose reduction required. For CrCl <60 mL/min, halve the dose. For CrCl <30 mL/min, use 1/4th of the usual dose. Not recommended for severe impairment.
How to Take
Tysul tablets are for oral administration. They can be taken with or without food. To avoid potential sleep disturbances, it is advisable not to take the last dose late in the evening.
Mechanism of Action
Sulpiride acts as a selective antagonist of dopamine D2 receptors in the limbic system, leading to its antipsychotic, antiemetic, and anxiolytic effects. At lower doses, it can block presynaptic D2 receptors, enhancing dopaminergic transmission.
Pharmacokinetics
Onset
Clinical effects may take days to weeks for psychiatric conditions; antiemetic effects may be quicker.
Excretion
Mainly excreted unchanged in urine and feces.
Half life
Elimination half-life is approximately 7-9 hours, prolonged in renal impairment.
Absorption
Oral absorption is moderate (25-35%), reaching peak plasma concentrations in 3-6 hours. Food may slightly reduce absorption.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to Sulpiride or excipients
- Phaeochromocytoma
- Prolactin-dependent tumors (e.g., breast cancer, pituitary adenoma)
- Acute porphyria
- Concomitant use with levodopa
Drug Interactions
Levodopa
Antagonistic effect, contraindicated due to reciprocal antagonism.
Antacids and Sucralfate
May reduce the absorption of Sulpiride; administer at least 2 hours apart.
Alcohol and CNS depressants
Increased sedative effect.
Hypokalaemic agents (e.g., diuretics)
Increase the risk of QT prolongation; correct potassium levels.
Drugs prolonging QT interval (e.g., antiarrhythmics, tricyclic antidepressants)
Increased risk of ventricular arrhythmias, including Torsade de Pointes.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, blurred vision, dry mouth, tachycardia, hypotension, extrapyramidal symptoms, and possibly coma. Treatment is symptomatic and supportive, including monitoring vital signs and ECG.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Sulpiride is excreted in breast milk; therefore, breastfeeding is not recommended during treatment.
