Description
Overview of the medicine
Ufenac-SR 200 mg Tablet contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and joint stiffness caused by arthritis. It is formulated as a sustained-release tablet for prolonged action.
Uses & Indications
Dosage
Adults
One Ufenac-SR 200 mg tablet once daily, preferably with food.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential for increased adverse effects.
Renal_impairment
Use with caution; dosage adjustment may be necessary in severe impairment. Not recommended for patients with advanced renal disease.
How to Take
Swallow the tablet whole with a glass of water, preferably during or after a meal. Do not crush, chew, or break the tablet.
Mechanism of Action
Diclofenac Sodium works by inhibiting the activity of cyclooxygenase (COX) enzymes, particularly COX-1 and COX-2. This inhibition leads to a reduction in the synthesis of prostaglandins, which are key mediators of inflammation, pain, and fever.
Pharmacokinetics
Onset
Pain relief may begin within 1 hour, but full effect for chronic conditions takes longer.
Excretion
Approximately 60% excreted in urine as metabolites, and 35% in bile/feces.
Half life
Approximately 1-2 hours (terminal half-life of parent compound), but effective duration is prolonged by SR formulation.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached within 4-5 hours due to sustained-release formulation.
Metabolism
Extensive hepatic metabolism, primarily by hydroxylation and glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to diclofenac or other NSAIDs
- Active peptic ulcer or gastrointestinal bleeding
- History of asthma, urticaria, or allergic reactions after taking aspirin or other NSAIDs
- Severe heart failure, renal failure, or hepatic failure
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery
Drug Interactions
SSRIs
Increased risk of GI bleeding.
Lithium and Methotrexate
Increased plasma levels of lithium and methotrexate, leading to toxicity.
Cyclosporine and Tacrolimus
Increased nephrotoxicity.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Diuretics and Antihypertensives
Reduced efficacy of diuretics and antihypertensives.
Other NSAIDs or corticosteroids
Increased risk of gastrointestinal side effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, GI bleeding, hypotension, respiratory depression, and coma. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal. Dialysis is generally not effective.
Pregnancy & Lactation
Diclofenac is contraindicated during the third trimester of pregnancy due to potential fetal harm (e.g., premature closure of ductus arteriosus). Use during early pregnancy and lactation only if clearly needed and benefits outweigh risks. Consult a doctor.
