Description
Overview of the medicine
Fluconazole is an antifungal medication used to treat a variety of fungal infections, including candidiasis (yeast infections) of the mouth, throat, esophagus, abdomen, lungs, blood, and other organs. It is also used to treat meningitis (inflammation of the membranes surrounding the brain and spinal cord) caused by fungi and to prevent yeast infections in patients undergoing chemotherapy or radiation treatment before a bone marrow transplant.
Uses & Indications
Dosage
Adults
For certain severe fungal infections or specific dermatophyte infections: 250 mg once daily or as directed by physician. For vaginal candidiasis (single dose): 150 mg. Specific regimens might use 250 mg for refractory cases.
Elderly
Dosage should be adjusted based on renal function.
Renal_impairment
Dose adjustment required. For creatinine clearance < 50 mL/min, reduce dose by 50%.
How to Take
Take orally with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14-alpha-demethylase, which is necessary for the conversion of lanosterol to ergosterol. Ergosterol is a vital component of the fungal cell membrane, and its impaired synthesis leads to increased cellular permeability and eventual cell lysis.

Side Effects
Contraindications
- •Hypersensitivity to fluconazole or other azole antifungals
- •Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin due to risk of QTc prolongation.
Drug Interactions
Statins
Increased risk of myopathy/rhabdomyolysis.
Rifampin
Decreased fluconazole levels.
Warfarin
Increased prothrombin time.
Phenytoin
Increased phenytoin levels.
Benzodiazepines
Increased benzodiazepine levels.
Oral Hypoglycemics
Increased risk of hypoglycemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Treatment is symptomatic and supportive. Gastric lavage may be considered. Hemodialysis significantly reduces plasma levels.
Pregnancy & Lactation
Pregnancy Category C (for single dose vaginal candidiasis); Category D (for prolonged use in severe or life-threatening infections). Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, caution advised.
