Description
Overview of the medicine
Rifaximin is a non-systemic broad-spectrum antibiotic used to treat various gastrointestinal conditions caused by bacteria due to its localized action in the gut.
Uses & Indications
Dosage
Adults
For Traveler's Diarrhea: 200 mg (approx. 10 ml of 100 mg/5 ml suspension) orally three times daily for 3 days. For Hepatic Encephalopathy: 550 mg (approx. 27.5 ml of 100 mg/5 ml suspension) orally twice daily.
Elderly
No specific dosage adjustment required based on age, due to minimal systemic absorption.
Renal_impairment
No dosage adjustment required due to minimal systemic absorption and fecal excretion.
How to Take
Oral suspension. Shake well before use. Can be taken with or without food. Use a measuring device for accurate dosing.
Mechanism of Action
Inhibits bacterial RNA synthesis by binding to the beta subunit of bacterial DNA-dependent RNA polymerase, leading to bacteriostatic or bactericidal effects.
Pharmacokinetics
Onset
Varies by condition; typically within 24-48 hours for diarrhea symptom improvement.
Excretion
Primarily excreted unchanged (approx. 96%) in feces.
Half life
Plasma half-life is approximately 5-6 hours.
Absorption
Very low systemic absorption (<0.4%) after oral administration.
Metabolism
Minimally metabolized by CYP3A4 enzymes in the liver.
Side Effects
Contraindications
- Hypersensitivity to rifaximin, other rifamycins, or any component of the formulation.
- Patients with diarrhea complicated by fever or blood in the stool, or severe diarrhea.
Drug Interactions
Warfarin
Potential for altered International Normalized Ratio (INR); close monitoring of INR is recommended.
P-glycoprotein inhibitors (e.g., cyclosporine)
May increase systemic exposure of rifaximin, though this is usually clinically insignificant due to low baseline absorption.
Storage
Store below 30°C (86°F), protected from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Due to minimal systemic absorption, overdose is unlikely to cause severe systemic effects.
Pregnancy & Lactation
Pregnancy Category C: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: It is unknown whether rifaximin is excreted in human milk. Exercise caution when administering to a nursing mother.

- Liver function tests (for patients with hepatic impairment, especially for hepatic encephalopathy indication)
- International Normalized Ratio (INR) for patients concurrently on warfarin.
Doctor Notes
- Rifaximin is a gut-specific antibiotic with minimal systemic absorption, making it an excellent choice for localized gastrointestinal infections. Emphasize patient compliance to complete the entire prescribed course to ensure eradication and prevent resistance.
- Advise patients to seek medical attention if diarrhea does not improve within 24-48 hours or worsens, or if they develop fever or bloody stools.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Do not use if diarrhea is accompanied by fever, blood in the stool, or if diarrhea worsens.
- Report any severe or persistent side effects to your doctor immediately.
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and continue your regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness in some individuals. Avoid driving or operating heavy machinery if you experience dizziness or other central nervous system effects.
