Description
Overview of the medicine
Ulticort 0.05% Cream contains Clobetasol Propionate, a very potent corticosteroid, used for the short-term treatment of inflammatory and pruritic manifestations of moderate to severe corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Treatment should be limited to 2-4 weeks. If no improvement is seen, reassess diagnosis.
Elderly
Use with caution due to increased skin fragility and potential for systemic absorption. Dosage as adults, but limit duration.
Renal_impairment
No specific dose adjustment is generally required for topical use, but caution is advised in extensive use due to potential systemic absorption.
How to Take
For external use only. Apply a thin film to the affected skin area and rub in gently. Do not use on face, groin, or axillae unless specifically directed by a physician. Wash hands after application.
Mechanism of Action
Clobetasol Propionate is a high-potency corticosteroid that acts by inducing phospholipase A2 inhibitory proteins, lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Clinical effects may be observed within hours to a few days.
Excretion
Excreted primarily by the kidneys, with some biliary excretion.
Half life
Plasma half-life of absorbed clobetasol propionate is approximately 4 hours.
Absorption
Systemic absorption of topical corticosteroids can occur, especially through denuded or abraded skin, or under occlusive dressings. Absorption is proportional to potency and duration of exposure.
Metabolism
Primarily metabolized in the liver to inactive metabolites, similar to other corticosteroids.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the cream
- Untreated cutaneous infections (bacterial, fungal, viral, parasitic)
- Acne vulgaris, rosacea, perioral dermatitis
- Pruritus without inflammation
- Children under 12 years of age (due to potential for systemic effects)
Drug Interactions
Corticosteroids (systemic)
Concurrent use may increase the risk of systemic corticosteroid adverse effects.
CYP3A4 inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol Propionate, potentially leading to increased systemic side effects.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely with topical use. Chronic overdose or misuse can lead to systemic effects such as Cushing's syndrome, hyperglycemia, and adrenal suppression. Treatment involves gradual withdrawal of the corticosteroid under medical supervision and symptomatic management.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid extensive use or prolonged treatment during pregnancy and lactation. Exercise caution when administered to a nursing woman.

Clinical Trials
Clobetasol Propionate has been extensively studied in numerous clinical trials demonstrating its efficacy in treating various steroid-responsive dermatoses. Studies focus on its potent anti-inflammatory effects and comparisons with other corticosteroids.
Lab Monitoring
- For prolonged or extensive use, or in children, periodic monitoring for HPA axis suppression may be warranted via ACTH stimulation test or plasma cortisol levels.
Doctor Notes
- This is a potent topical corticosteroid. Prescribe for short-term use only (max 2-4 weeks).
- Avoid use on the face, groin, axillae, or for diaper rash due to increased absorption and risk of side effects.
- Counsel patients on proper application technique and warning signs of local/systemic side effects.
Patient Guidelines
- Use only as directed by your doctor and do not exceed the prescribed duration of treatment (usually 2-4 weeks).
- Do not apply to the face, groin, or underarms unless specifically instructed by your doctor.
- Avoid applying cream to large areas of the body or under occlusive dressings, as this can increase systemic absorption.
- Wash your hands thoroughly after applying the cream.
Missed Dose Advice
If a dose is missed, apply it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
