Description
Overview of the medicine
Human Albumin 20% Injection is a sterile solution of human albumin, a natural protein found in blood plasma. It is used to restore and maintain circulating blood volume where blood volume deficiency is evident.
Uses & Indications
Dosage
Adults
Dosage varies greatly depending on the patient's condition, weight, and clinical response. Typically, 50-100g may be administered daily or as required to achieve desired oncotic pressure/volume. Infusion rate should be adjusted according to the patient's circulatory status.
Elderly
No specific dose adjustment is generally required, but close monitoring for signs of circulatory overload is recommended.
Renal_impairment
No specific dose adjustment is typically required, but use with caution and monitor for fluid overload, especially in anuric patients.
How to Take
Administered intravenously, undiluted, or diluted in isotonic solution. The infusion rate should be adjusted according to individual circumstances and indication, generally not exceeding 1-2 ml/minute for 20% solution.
Mechanism of Action
Albumin contributes to the oncotic pressure of plasma and is essential for the transport of various molecules (e.g., hormones, enzymes, drugs). It increases plasma volume by drawing interstitial fluid into the vascular space, thereby supporting circulatory function.
Pharmacokinetics
Onset
Immediate.
Excretion
Excreted as peptides and amino acids which are then recycled or excreted via kidneys.
Half life
Approximately 15-19 days, but the initial volume expansion effect is transient.
Absorption
Administered intravenously, providing immediate and complete bioavailability.
Metabolism
Catabolized by body cells into peptides and amino acids.
Side Effects
Contraindications
- Hypersensitivity to albumin preparations or any excipient.
- Severe anemia or cardiac failure with normal or increased intravascular volume.
- Patients at risk of hypervolemia or pulmonary edema.
Drug Interactions
None specific
No significant drug-drug interactions are generally reported with human albumin.
ACE Inhibitors
Potential for increased bradykinin levels, but clinical significance with albumin is generally low.
Storage
Store below 30°C. Do not freeze. Protect from light. Do not use if the solution is turbid or contains particles.
Overdose
Overdose can lead to hypervolemia, characterized by signs of circulatory overload such as pulmonary edema, elevated central venous pressure, and headache. Management involves immediately stopping the infusion and initiating symptomatic treatment, possibly including diuretics.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed. Human albumin is a normal constituent of human blood, and its use during pregnancy or lactation is considered safe when clinically indicated, as it is not expected to cause adverse effects to the fetus or breastfed infant.

Doctor Notes
- Careful monitoring of hemodynamic parameters (blood pressure, heart rate, CVP) and fluid balance is crucial during administration.
- Adjust infusion rate based on patient response and clinical status to avoid circulatory overload.
- Assess patient's allergic history before administration.
Patient Guidelines
- Report any unusual symptoms or discomfort immediately to the healthcare professional.
- Remain under medical supervision during and after administration.
- Understand the purpose of the treatment and potential side effects.
Missed Dose Advice
Human albumin is administered by a healthcare professional in a hospital setting; therefore, missed doses are not typically a concern for the patient.
Driving Precautions
Not applicable as this medication is typically administered in an acute hospital setting.
Lifestyle Advice
- Maintain good hydration as advised by the healthcare provider.
