Uses & Indications
Dosage
Adults
25 mg once daily, preferably after food. Dosage may be adjusted based on patient response and tolerability, not exceeding 25 mg daily.
Elderly
Lower doses may be considered due to potential for increased sensitivity and risk of adverse effects. Careful monitoring of renal function is advised.
Renal_impairment
Use with caution. Dosage reduction may be necessary in patients with mild to moderate renal impairment. Contraindicated in severe renal impairment.
How to Take
Take the capsule orally, preferably with food or milk to minimize gastrointestinal irritation. Swallow whole with a glass of water, do not chew or crush.
Mechanism of Action
Umethacin, an oxicam derivative, primarily acts by inhibiting cyclooxygenase (COX) enzymes, particularly COX-1 and COX-2. This inhibition leads to a reduction in the synthesis of prostaglandins, which are key mediators of pain, inflammation, and fever.

Excretion
Mainly excreted in the urine (both as metabolites and unchanged drug) and to a lesser extent in feces.

Elimination half-life is relatively long, often ranging from 24 to 48 hours, supporting once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are typically reached within 2-4 hours.
Metabolism

Contraindications

Drug Interactions
Lithium

Diuretics and ACE Inhibitors
May reduce antihypertensive and diuretic effects; increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, lethargy, drowsiness, headache, dizziness, tinnitus, and convulsions. Management is symptomatic and supportive. Gastric lavage or activated charcoal may be considered if recent ingestion. Monitor renal and hepatic function.
Pregnancy & Lactation
Pregnancy Category C in early pregnancy, Category D in the third trimester. Avoid during the third trimester due to potential premature closure of the fetal ductus arteriosus. Use only if potential benefit justifies the potential risk to the fetus in early pregnancy. Not recommended during breastfeeding as NSAIDs can pass into breast milk.
