Overview of the medicine
Umodin 10 mg tablet contains Betahistine dihydrochloride, used for the symptomatic treatment of Meniere's disease, characterized by episodes of vertigo, tinnitus, and hearing loss. It helps to reduce the frequency and severity of these symptoms.
Uses & Indications
Dosage
Adults
Typically 8-16 mg three times daily, taken with meals. Dosage may be adjusted based on clinical response, usually starting with 8 mg three times daily and increasing to 16 mg three times daily if needed. For 10 mg tablet, one tablet thrice daily.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment, but caution is advised.
How to Take
Take tablets orally, preferably with meals to minimize gastric discomfort. Swallow with water.
Mechanism of Action
Betahistine acts as a weak histamine H1 receptor agonist and a potent histamine H3 receptor antagonist in both the peripheral and central vestibular systems. This action is believed to increase blood flow in the inner ear by relaxing pre-capillary sphincters, improving microcirculation. It also modulates neurotransmitter release, particularly histamine, in the brainstem, thereby reducing vestibular imbalance.

Onset of action varies; symptomatic improvement may be observed within hours to days, full effect often takes weeks.
Excretion
Excreted primarily in the urine as 2-PAA within 24 hours.
Half life
Approximately 3-4 hours for the main metabolite (2-pyridylacetic acid).
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1 hour.
Metabolism
Rapidly and almost completely metabolized by first-pass metabolism to 2-pyridylacetic acid (2-PAA), which is the primary metabolite.
Side Effects
Contraindications

- •Pheochromocytoma (a tumor of the adrenal gland)
Drug Interactions
Antihistamines
May reduce the therapeutic effect of betahistine.
Beta-2 Agonists
No clinically relevant interactions reported.
Monoamine Oxidase Inhibitors (MAOIs)
MAOIs may inhibit the metabolism of betahistine, leading to increased exposure. Caution is advised.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, drowsiness, and convulsions at very high doses. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if appropriate.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether betahistine is excreted in human milk; use with caution in breastfeeding mothers and consult a doctor.
