Description
Overview of the medicine
Unipril 25 mg Tablet contains Lisinopril, an Angiotensin-Converting Enzyme (ACE) inhibitor. It is used to treat high blood pressure (hypertension), heart failure, and to improve survival after a heart attack (acute myocardial infarction). It works by relaxing blood vessels and making it easier for your heart to pump blood.
Uses & Indications
Dosage
Adults
Hypertension: Initial 10 mg once daily; usual maintenance 20-40 mg once daily. Heart Failure: Initial 2.5-5 mg once daily; usual maintenance 5-20 mg once daily. Post-MI: Initial 5 mg, then 5 mg after 24 hours, 10 mg after 48 hours, then 10 mg once daily.
Elderly
Lower initial dose may be considered, especially with impaired renal function. Usually, no specific dosage adjustment is required based on age alone.
Renal_impairment
Initial dose should be adjusted based on creatinine clearance (CrCl). For CrCl 10-30 mL/min, initial dose 2.5-5 mg once daily. For CrCl <10 mL/min, initial dose 2.5 mg once daily.
How to Take
Take Unipril 25 mg Tablet orally once daily, with or without food. It is best to take it at the same time each day.
Mechanism of Action
Lisinopril inhibits the Angiotensin-Converting Enzyme (ACE), which is responsible for converting Angiotensin I to Angiotensin II. Angiotensin II is a potent vasoconstrictor and stimulates aldosterone secretion. By inhibiting ACE, Lisinopril reduces vasoconstriction, lowers aldosterone levels, leading to decreased blood pressure and reduced cardiac preload and afterload.
Pharmacokinetics
Onset
Within 1 hour for blood pressure lowering effect.
Excretion
Excreted almost exclusively by the kidneys as the unchanged drug.
Half life
Effective half-life for accumulation is 12 hours. Terminal half-life is approximately 30 hours.
Absorption
Approximately 25-30% of an oral dose is absorbed, with variability. Absorption is not affected by food.
Metabolism
Lisinopril is not metabolized and is excreted unchanged.
Side Effects
Contraindications
- •History of angioedema related to previous ACE inhibitor treatment
- •Hereditary or idiopathic angioedema
- •Pregnancy (especially second and third trimesters)
- •Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²)
- •Hypersensitivity to lisinopril or any other ACE inhibitor
Drug Interactions
Lithium
Increased serum lithium concentrations and lithium toxicity.
Aliskiren
Concomitant use is contraindicated in patients with diabetes or renal impairment due to increased risk of hyperkalemia, hypotension, and renal impairment.
Diuretics
Increased risk of symptomatic hypotension, especially after initiating lisinopril.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of lisinopril and increase the risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypotension, electrolyte disturbances (hyperkalemia), and renal failure. Treatment is symptomatic and supportive. Intravenous saline may be administered to restore blood pressure. Lisinopril can be removed from the circulation by hemodialysis.
Pregnancy & Lactation
Unipril is contraindicated in the second and third trimesters of pregnancy due to risk of fetal injury and death. It is not recommended during the first trimester. Use in lactation is not recommended as it is excreted in breast milk.
