Overview of the medicine
Uniquin 250 mg Tablet contains Ofloxacin, a synthetic fluoroquinolone antibacterial agent. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
Typical dose is 250 mg to 500 mg every 12 hours, depending on the type and severity of infection. For most infections, 250 mg twice daily is common.
Elderly
Dosage adjustment may be necessary based on renal function. Generally, the lowest effective dose should be used.
Renal_impairment
Dosage adjustment required. For creatinine clearance 20-50 mL/min, the dose should be reduced to 125-250 mg every 24 hours. For <20 mL/min, 125 mg every 24 hours or 250 mg every 48 hours.
How to Take
Take Uniquin 250 mg Tablet orally, with or without food. Swallow the tablet whole with water. Do not crush, chew, or break it. Avoid taking antacids, iron supplements, or dairy products within 2 hours of taking Ofloxacin.
Mechanism of Action
Ofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and ultimately cell death.
Pharmacokinetics
Onset
Typically within 1-2 hours of administration.
Excretion
Primarily excreted unchanged in the urine (70-80% of the dose) via glomerular filtration and tubular secretion. Small amounts are excreted in feces.
Half life
Approximately 4-6 hours, allowing for twice-daily dosing.
Absorption
Rapidly and well-absorbed from the gastrointestinal tract after oral administration, with bioavailability around 98%. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Minimally metabolized in the liver (less than 5% of dose).
Side Effects
Contraindications
- Hypersensitivity to Ofloxacin or other fluoroquinolones
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy or other seizure disorders
- Children and adolescents (under 18 years) due to risk of joint damage
- Patients with myasthenia gravis (may exacerbate symptoms)
Drug Interactions
Warfarin
Ofloxacin may potentiate the anticoagulant effect of Warfarin, increasing the risk of bleeding. Monitor INR closely.
Theophylline
Ofloxacin can increase plasma levels of Theophylline, leading to increased risk of toxicity.
Corticosteroids
Concomitant use with corticosteroids may increase the risk of tendon rupture.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Co-administration with NSAIDs may increase the risk of CNS stimulation and convulsive seizures.
Antacids (containing magnesium, aluminum), Sucralfate, Iron/Zinc supplements
Can significantly reduce the absorption of Ofloxacin. Administer Ofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include CNS symptoms such as confusion, dizziness, impaired consciousness, convulsive seizures, and gastrointestinal reactions like nausea and erosion of the gastric mucosa. In case of overdose, gastric lavage may be considered. Symptomatic and supportive treatment should be initiated. Monitor vital signs and ECG.
Pregnancy & Lactation
Pregnancy Category C. Ofloxacin should not be used during pregnancy unless the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk and is therefore not recommended during breastfeeding due to potential harm to the infant, including interference with cartilage development.
