Overview of the medicine
Uricare-SR 0.4 mg Capsule contains Tamsulosin Hydrochloride, an alpha-1 blocker used to treat the signs and symptoms of Benign Prostatic Hyperplasia (BPH). It helps relax the muscles in the prostate and bladder neck, improving urine flow.
Uses & Indications
Dosage
Adults
0.4 mg once daily, taken approximately 30 minutes after the same meal each day. May be increased to 0.8 mg once daily after 2-4 weeks if necessary.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is generally required for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment (CrCl < 10 mL/min).
How to Take
Take orally once daily, approximately 30 minutes after the same meal each day. The capsule should be swallowed whole; do not crush, chew, or open it.
Mechanism of Action
Tamsulosin selectively and competitively blocks alpha-1A and alpha-1D adrenergic receptors located in the prostatic stromal smooth muscle, bladder neck, and prostatic urethra. This blockade causes relaxation of the smooth muscles in these areas, thereby decreasing urethral resistance and improving urine flow and reducing BPH symptoms.

Onset
Clinical effects seen within 4-8 hours; full effect may take 2-4 weeks.
Excretion
Excreted mainly in the urine (76% of dose) and faeces (21% of dose) as metabolites.
Half life
Approximately 9-15 hours in healthy volunteers, and around 14-15 hours in the target population.
Absorption
Well absorbed from the gastrointestinal tract, especially after food. Bioavailability is nearly 90%.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2D6 enzymes.

Contraindications
- Hypersensitivity to tamsulosin hydrochloride or any component of the formulation
- History of orthostatic hypotension
- Severe hepatic impairment
Drug Interactions
Other alpha-adrenergic blockers
Increased risk of hypotension. Should not be used in combination.
Strong CYP2D6 inhibitors (e.g., paroxetine)
May increase tamsulosin exposure. Concomitant use should be used with caution.
PDE5 inhibitors (e.g., sildenafil, tadalafil)
Increased risk of hypotension, especially orthostatic hypotension. Use with caution.
