Overview of the medicine
Fosfomycin is an antibiotic used to treat uncomplicated urinary tract infections (UTIs) in women. It works by inhibiting bacterial cell wall synthesis. This single-dose oral solution is convenient for acute cystitis.
Uses & Indications
Dosage
Adults
One single dose of 1500 mg (contents of one sachet) dissolved in water, usually at bedtime after emptying the bladder.
Elderly
No specific dose adjustment is needed based on age alone, but renal function should be assessed and considered.
Renal_impairment
Not recommended for patients with severe renal insufficiency (creatinine clearance < 10 mL/min). Use with caution in moderate impairment.
How to Take
Dissolve the contents of the sachet in 1/2 to 1 cup (120-240 mL) of water (not hot water). Stir to dissolve and drink immediately. May be taken with or without food. It is recommended to take it at bedtime after emptying the bladder for best results.
Mechanism of Action
Fosfomycin inhibits bacterial cell wall synthesis by inactivating enolpyruvyl transferase, an enzyme crucial for peptidoglycan formation, thereby exerting bactericidal effects.
Pharmacokinetics
Onset
Peak plasma concentration is reached within 2-4 hours, with urinary concentrations remaining therapeutic for over 24-48 hours.
Excretion
Primarily excreted unchanged in the urine (about 50%) and feces (about 18%) within 72 hours.
Half life
Approximately 4-6 hours in plasma, but longer in urine.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration. Bioavailability of fosfomycin trometamol is approximately 37%.
Metabolism
Not extensively metabolized in the body; primarily excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to fosfomycin or any excipients.
- •Severe renal insufficiency (creatinine clearance < 10 mL/min).
- •Patients undergoing hemodialysis.
Drug Interactions
Metoclopramide
Concomitant use with metoclopramide may reduce serum concentrations and urinary excretion of fosfomycin, decreasing its efficacy. Co-administration should be avoided.
Other prokinetic agents
Similar interactions may occur with other drugs that accelerate gastrointestinal motility.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include diarrhea, abdominal discomfort, and electrolyte disturbances. Management is symptomatic and supportive. There is no specific antidote. Hydration may aid urinary excretion.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm, but human data is limited. Use only if clearly needed after consulting a doctor. Excreted in human milk; use with caution during lactation, only if potential benefits outweigh risks.
