Description
Overview of the medicine
Urikal 1500 mg Oral Solution contains potassium citrate, used to treat certain kidney stone conditions and metabolic acidosis. It works by making the urine less acidic, which helps prevent the formation of new kidney stones and can dissolve existing uric acid stones.
Uses & Indications
Dosage
Adults
Typically 10-20 mEq (1100-2200 mg) three to four times daily, adjusted to maintain urinary pH between 6.0-7.0. For 1500mg (approx 15 mEq), it could be 1-2 pouches daily.
Elderly
Similar to adult dosage, with careful monitoring of renal function.
Renal_impairment
Contraindicated in severe renal impairment. In mild to moderate impairment, use with caution and frequent monitoring of serum electrolytes and renal function.
How to Take
Dissolve the contents of the sachet in a glass of water (100-200 ml) and take orally, preferably with or after meals. Drink plenty of water throughout the day.
Mechanism of Action
Potassium citrate is metabolized in the body to bicarbonate, which increases the pH of the urine and raises urinary citrate levels. Elevated urinary citrate binds with calcium, preventing its crystallization, while the increased pH helps in the dissolution of uric acid stones and prevents new formation.
Pharmacokinetics
Onset
Within hours of administration, urinary pH changes become apparent.
Excretion
Excreted primarily via the kidneys as citrate or after metabolism as bicarbonate.
Half life
Approximately 20-30 minutes (for citrate, potassium kinetics differ slightly)
Absorption
Readily absorbed from the gastrointestinal tract.
Metabolism
Citrate is metabolized to bicarbonate in the liver.
Side Effects
Contraindications
- Severe renal impairment (glomerular filtration rate less than 0.7 ml/min/1.73m2)
- Untreated Addison's disease
- Hyperkalemia or conditions predisposing to hyperkalemia (e.g., severe myocardial damage, acute dehydration)
- Active urinary tract infection
- Gastric ulceration
- Bowel obstruction
Drug Interactions
Anticholinergics
May increase the risk of gastrointestinal irritation due to slower transit time.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce renal potassium excretion and increase risk of hyperkalemia.
ACE inhibitors and Angiotensin Receptor Blockers (ARBs)
May increase serum potassium levels.
Potassium-sparing diuretics (e.g., spironolactone, triamterene)
Increased risk of hyperkalemia.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hyperkalemia and metabolic alkalosis. Management involves discontinuing the drug, monitoring serum electrolytes, and providing supportive care. In severe cases, intravenous calcium gluconate, insulin/glucose, or hemodialysis may be required for hyperkalemia.
Pregnancy & Lactation
Use with caution during pregnancy only if clearly needed and the benefits outweigh the risks. Limited data on excretion in breast milk; use with caution in lactating mothers.
