Overview of the medicine
Urinor 1500 mg Oral Solution contains Fosfomycin Trometamol, an antibiotic used for the treatment of acute uncomplicated urinary tract infections (UTIs) in women. It is typically administered as a single oral dose.
Uses & Indications
Dosage
Adults
The usual dose for acute uncomplicated UTIs in women is one single sachet (1500 mg) of Urinor oral solution.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function.
Renal_impairment
Fosfomycin is contraindicated in patients with severe renal insufficiency (creatinine clearance <10 mL/min).
How to Take
Dissolve the contents of one sachet in 1/2 to 1 glass (120-240 mL) of water (not hot water). Stir until completely dissolved. Drink immediately after dissolving. It is best to take on an empty stomach, at least 2-3 hours after a meal, preferably at bedtime after emptying the bladder.
Mechanism of Action
Fosfomycin inhibits bacterial cell wall synthesis by inactivating the enzyme UDP-N-acetylglucosamine-3-enolpyruvyltransferase (MurA), which is critical for the first step of peptidoglycan synthesis in the bacterial cell wall. This leads to bactericidal action.
Pharmacokinetics
Onset
Peak plasma concentrations reached in 2-4 hours. Excreted mostly in active form in urine, achieving high concentrations in the bladder.
Excretion
Mainly excreted unchanged in the urine (about 50% of the dose) and feces (about 18% of the dose) within 24-48 hours.
Half life
Approximately 4 hours.
Absorption
Rapidly absorbed after oral administration and converted to free fosfomycin. Bioavailability is about 30-50%. Food can delay absorption and reduce peak plasma concentrations.
Metabolism
Minimal hepatic metabolism. Primarily excreted unchanged.

Contraindications
- Hypersensitivity to fosfomycin or any excipients.
- Severe renal insufficiency (creatinine clearance <10 mL/min) and patients undergoing hemodialysis.
Drug Interactions
Metoclopramide
Concomitant use with metoclopramide (a prokinetic agent) may decrease serum concentrations and urinary excretion of fosfomycin. It is recommended to avoid co-administration or administer fosfomycin 2-3 hours before metoclopramide.
Other Antibiotics
No significant adverse interactions reported with other commonly used antibiotics for UTIs.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.

Symptoms of overdose may include vestibular disorders, impaired hearing, metallic taste, and general decrease in renal function. Treatment should be symptomatic and supportive. Enhanced elimination of fosfomycin by hemodialysis is possible.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Should be used during pregnancy only if clearly needed after consulting a doctor. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing woman.
