Description
Overview of the medicine
Solifenacin is an anticholinergic medication used to treat symptoms of overactive bladder (OAB), such as urinary urgency, frequency, and urge incontinence.
Uses & Indications
Dosage
Adults
The usual recommended dose for solifenacin is 5 mg once daily, which may be increased to 10 mg once daily based on individual response and tolerability. A 25 mg dose is not a standard recommended dose for Solifenacin.
Elderly
No dose adjustment is generally required, but caution is advised in patients with severe renal or hepatic impairment.
Renal_impairment
In patients with severe renal impairment (creatinine clearance < 30 mL/min), the maximum dose should not exceed 5 mg once daily.
How to Take
Uroblock 25 mg Tablet should be taken orally once daily, with or without food. It should be swallowed whole with some liquid and not chewed or crushed.
Mechanism of Action
Solifenacin selectively blocks M3 muscarinic cholinergic receptors in the bladder, reducing detrusor muscle contraction and thereby improving bladder capacity and reducing involuntary contractions.
Pharmacokinetics
Onset
Clinical effects typically observed within a few weeks, with full effect after 4-8 weeks.
Excretion
Approximately 69% excreted in urine and 23% in feces.
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract; peak plasma concentrations reached after 3-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 3A4 (CYP3A4) enzyme.
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Myasthenia gravis
- Severe hepatic impairment
- Hypersensitivity to solifenacin or any excipients.
Drug Interactions
Cholinergic Agonists
May reduce the effect of cholinergic agonists.
Other Anticholinergic Drugs
May enhance anticholinergic side effects.
CYP3A4 Inducers (e.g., Rifampicin)
May decrease solifenacin concentrations.
Potent CYP3A4 Inhibitors (e.g., Ketoconazole)
Co-administration with potent CYP3A4 inhibitors like ketoconazole may increase solifenacin exposure; max solifenacin dose should be 5 mg/day.
Drugs affecting gastrointestinal motility (e.g., Metoclopramide, Cisapride)
Solifenacin may reduce the effect of these drugs due to decreased gastric motility.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe anticholinergic effects such as severe dry mouth, blurred vision, tachycardia, urinary retention, and dilated pupils. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if solifenacin is excreted in human breast milk; use with caution during lactation.

Lab Monitoring
- No routine laboratory monitoring is required. Assess renal and hepatic function if impairment is suspected or in specific patient populations.
Doctor Notes
- Advise patients on the potential for constipation and dry mouth, and strategies for management.
- Monitor patients for signs of urinary retention, especially those with bladder outlet obstruction.
- Counsel patients on potential effects on driving and operating machinery.
