Description
Overview of the medicine
Alfuzosin is an alpha-1 adrenergic blocker used to treat the symptoms of benign prostatic hyperplasia (BPH) by relaxing muscles in the prostate and bladder neck, thereby improving urine flow and reducing bladder outflow obstruction.
Uses & Indications
Dosage
Adults
10 mg once daily, immediately after the same meal each day.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential for increased sensitivity.
Renal_impairment
Mild to moderate renal impairment (CrCl ≥30 mL/min): No dose adjustment needed. Severe renal impairment (CrCl <30 mL/min): Use with caution, consider alternative treatments.
How to Take
Swallow the tablet whole with water, do not crush, chew, or divide. Take immediately after the same meal each day.
Mechanism of Action
Alfuzosin selectively blocks alpha-1 adrenergic receptors located in the prostate, bladder base, bladder neck, prostatic capsule, and prostatic urethra. This blockade causes relaxation of the smooth muscle in these tissues, reducing urethral resistance and improving urine flow in BPH patients.
Pharmacokinetics
Onset
Symptomatic relief within hours to days, full therapeutic effect may take several weeks.
Excretion
Mainly excreted in feces (75-91%), with a smaller amount (approximately 11%) excreted in urine.
Half life
Approximately 5 to 8 hours (for the sustained-release formulation).
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached about 8 hours post-dose. Absolute bioavailability is approximately 64%. Administration after food may increase absorption.
Metabolism
Extensively metabolized in the liver, primarily via the CYP3A4 enzyme.
Side Effects
Contraindications
- Hypersensitivity to alfuzosin or any component of the formulation.
- Severe hepatic impairment.
- Concomitant administration of other alpha-1 blockers.
- Concomitant use of strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir).
- History of orthostatic hypotension.
Drug Interactions
Antihypertensive Drugs
May enhance the hypotensive effect of alfuzosin.
Other Alpha-1 Blockers
Increased risk of symptomatic hypotension.
PDE5 Inhibitors (e.g., sildenafil, tadalafil)
Concurrent use may lead to symptomatic hypotension, especially within 6 hours of taking alfuzosin.
Strong CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir)
Increase alfuzosin plasma concentrations, increasing risk of adverse effects.
