Description
Overview of the medicine
Uromax (Tamsulosin Hydrochloride) is an alpha1-adrenoceptor blocker used to treat the signs and symptoms of benign prostatic hyperplasia (BPH). It works by relaxing the muscles in the prostate and bladder neck, making urination easier and improving urinary flow.
Uses & Indications
Dosage
Adults
0.4 mg once daily, taken approximately 30 minutes after the same meal each day. May be increased to 0.8 mg once daily if response is inadequate after 2-4 weeks.
Elderly
No dosage adjustment is generally required. Monitor carefully for orthostatic hypotension.
Renal_impairment
No dosage adjustment is generally required for patients with mild to moderate renal impairment. Caution is advised for severe impairment (CrCl < 10 mL/min).
How to Take
Take orally, approximately 30 minutes after the same meal each day. Swallow the capsule whole; do not crush, chew, or open it.
Mechanism of Action
Tamsulosin selectively and competitively binds to alpha1A-adrenoceptors in the prostate, prostatic capsule, bladder neck, and prostatic urethra, leading to relaxation of the smooth muscle in these tissues. This reduces the outflow obstruction in the bladder, improving symptoms of BPH such as urinary urgency, hesitancy, and frequency.
Pharmacokinetics
Onset
Symptomatic improvement may be observed within 1-2 weeks of initiation of therapy.
Excretion
Primarily excreted in the urine (76%) and feces (21%) as metabolites, with less than 10% excreted unchanged in urine.
Half life
Approximately 9-13 hours in healthy volunteers and 14-15 hours in the target population (BPH patients).
Absorption
Rapidly absorbed from the gastrointestinal tract, nearly 100% bioavailability. Peak plasma concentrations are reached within 4-5 hours under fasting conditions, and 6-7 hours when taken with food.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2D6 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Tamsulosin or any component of the formulation
- •History of orthostatic hypotension
- •Severe hepatic impairment
Drug Interactions
Strong CYP2D6 inhibitors (e.g., paroxetine)
Concomitant use may increase Tamsulosin exposure.
PDE5 inhibitors (e.g., sildenafil, tadalafil)
Concomitant use may lead to additive blood pressure lowering effects and symptomatic hypotension. Administer with caution and maintain appropriate time interval.
Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
Concomitant use may significantly increase Tamsulosin exposure; use with caution.
Other alpha1-adrenoreceptor blockers (e.g., prazosin, terazosin)
Concomitant use may lead to enhanced hypotensive effects.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Hypotension (low blood pressure) may occur. Supportive treatment should be initiated, including placing the patient in a supine position and, if necessary, administering intravenous fluids and vasopressors. Renal function should be monitored.
Pregnancy & Lactation
Uromax is not indicated for use in women and should not be used during pregnancy or lactation.
