Overview of the medicine
Urotam 0.4 mg capsule contains Tamsulosin Hydrochloride, an alpha-blocker used to treat the symptoms of benign prostatic hyperplasia (BPH) such as difficulty urinating, painful urination, and frequent urination in men.
Uses & Indications
Dosage
Adults
0.4 mg once daily, administered approximately 30 minutes after the same meal each day. May be increased to 0.8 mg once daily after 2-4 weeks if necessary.
Elderly
No dosage adjustment required.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Swallow whole capsule with water, do not crush, chew, or open. Take approximately 30 minutes after the same meal each day.
Mechanism of Action
Tamsulosin selectively blocks alpha-1A adrenergic receptors in the prostate, prostatic capsule, bladder neck, and prostatic urethra. This blockade causes relaxation of the smooth muscles in these tissues, leading to improved urine flow and reduced symptoms of BPH.
Pharmacokinetics
Onset
Noticeable improvement in symptoms may take 1-2 weeks, but some effects can be seen within days.
Excretion
Mainly excreted in urine, primarily as metabolites.
Half life
Approximately 9-13 hours in healthy volunteers and 14-15 hours in the target population.
Absorption
Well absorbed from the gastrointestinal tract, nearly 90% bioavailability under fasting conditions.
Metabolism
Extensively metabolized by cytochrome P450 enzymes in the liver, primarily CYP3A4 and CYP2D6.
Side Effects
Contraindications

Drug Interactions
Other Alpha-Adrenergic Blockers
Increased risk of hypotension. Avoid concomitant use.
Strong CYP2D6 inhibitors (e.g., Paroxetine)

PDE5 Inhibitors (e.g., Sildenafil, Tadalafil)
May potentiate hypotensive effects. Use with caution, especially at initiation.
Strong CYP3A4 inhibitors (e.g., Ketoconazole)

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
May lead to severe hypotensive effects. Management is supportive, including lying the patient down and fluid replacement. Vasopressors may be used if needed.

Not indicated for use in women. Safety in pregnancy and lactation has not been established.
