Overview of the medicine
Ursodeoxycholic Acid (UDCA) is a bile acid used to dissolve cholesterol gallstones and treat certain liver diseases, such as primary biliary cholangitis (PBC).
Uses & Indications
Dosage
Adults
For gallstones: 8-10 mg/kg/day in 2-3 divided doses. For PBC: 13-15 mg/kg/day in 2-4 divided doses, adjusted based on patient response. Treatment typically continues until gallstone dissolution is confirmed or indefinitely for PBC.
Elderly
Similar to adult dosage; caution may be advised in patients with significant comorbidities, though no specific dose adjustment is usually needed.
Renal_impairment
No specific dose adjustment is required as Ursodeoxycholic Acid is primarily eliminated via bile and feces, not renally.
How to Take
Swallow the tablet whole with water. It is generally recommended to take with food or immediately after a meal to aid absorption and reduce gastrointestinal upset.
Mechanism of Action
UDCA reduces the cholesterol saturation of bile by suppressing hepatic cholesterol synthesis and secretion, thus facilitating the dissolution of cholesterol gallstones. It also protects hepatocytes from toxic bile acids and has immunomodulatory effects in chronic cholestatic liver diseases.

Onset
Gallstone dissolution can take months; for symptomatic relief in PBC, effects may be noticed within several weeks.
Excretion
Mainly excreted in feces via bile. A small amount is reabsorbed through the enterohepatic circulation.
Half life
Approximately 3.5-5.8 days (after continuous therapy, due to extensive enterohepatic recirculation).
Absorption
Well absorbed orally (approximately 90%), but undergoes significant first-pass metabolism.
Metabolism
Primarily conjugated with glycine or taurine in the liver, forming non-toxic conjugates.
Side Effects
Contraindications
- •Acute cholecystitis, cholangitis, or other acute inflammatory conditions of the gallbladder or bile ducts.
- •Obstruction of the biliary tract (e.g., common bile duct obstruction).
- •Calcified gallstones (radiopaque gallstones).
- •Impaired contractility of the gallbladder.
- •Allergy or hypersensitivity to Ursodeoxycholic Acid or any excipients.
Drug Interactions
Cyclosporine
UDCA may increase the absorption of cyclosporine. Cyclosporine levels should be monitored and dose adjusted if co-administered.
Cholestyramine, Colestipol, or Antacids containing aluminum hydroxide
These agents bind bile acids and reduce the absorption of UDCA. Administer UDCA at least 2 hours before or 4 hours after these agents.
Oestrogens, oral contraceptives, or lipid-lowering agents (e.g., clofibrate)
These drugs may increase cholesterol secretion into bile and could counteract the effect of UDCA.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose is rare. The most likely symptom of overdose is diarrhea. Treatment is symptomatic and supportive; fluid and electrolyte balance should be maintained.
Pregnancy & Lactation
Pregnancy Category B. While animal studies show no evidence of harm, human data are limited. Use only if clearly needed and potential benefits outweigh risks. Ursodeoxycholic Acid is excreted in breast milk in small amounts; use with caution during lactation and consult a doctor.
