Description
Overview of the medicine
Ursoglen contains Ursodeoxycholic Acid (UDCA), a naturally occurring bile acid. It is primarily used to dissolve cholesterol gallstones and to treat primary biliary cholangitis (PBC).
Uses & Indications
Dosage
Adults
Gallstone dissolution: 8-10 mg/kg/day, usually divided into 2-3 doses. PBC: 13-15 mg/kg/day, divided into 2-4 doses.
Elderly
No specific dose adjustment required, but monitor for side effects.
Renal_impairment
No dose adjustment required as UDCA is not significantly renally excreted.
How to Take
Take with food, preferably at bedtime for gallstone dissolution, or with meals for PBC.
Mechanism of Action
UDCA reduces cholesterol saturation of bile and slowly dissolves cholesterol gallstones. It also protects liver cells from toxic bile acids and has immunomodulatory effects in primary biliary cholangitis.
Pharmacokinetics
Onset
Variable; gallstone dissolution takes months.
Excretion
Excreted primarily in feces.
Half life
Approximately 3.5-5.8 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Undergoes extensive enterohepatic recirculation and conjugation with glycine and taurine in the liver.
Side Effects
Contraindications
- •Acute cholecystitis
- •Non-functioning gallbladder
- •Calcified gallstones
- •Biliary obstruction
- •Allergy to UDCA or other bile acids
Drug Interactions
Ciclosporin
UDCA may increase ciclosporin absorption. Monitor ciclosporin levels.
Estrogens, oral contraceptives, clofibrate
Can increase cholesterol secretion into bile, potentially counteracting UDCA's effect.
Cholestyramine, colestipol, aluminum hydroxide-containing antacids
May bind UDCA and reduce its absorption. Take UDCA at least 2 hours before or 4 hours after these agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Diarrhea may be the most prominent symptom. Treatment is supportive; no specific antidote. Discontinue therapy and manage symptoms.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed and potential benefits outweigh risks. UDCA is excreted in breast milk in small amounts; use with caution.
