Description
Overview of the medicine
Desloratadine is a non-drowsy antihistamine used to relieve symptoms of allergic conditions such as hay fever and chronic urticaria. It helps to reduce itching, sneezing, watery eyes, and runny nose.
Uses & Indications
Dosage
Adults
5 mg (10 mL oral solution) once daily.
Elderly
No specific dosage adjustment required, but caution is advised due to potential age-related decline in renal or hepatic function.
Renal_impairment
Initial dose of 5 mg every other day for severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
May be taken with or without food. Measure the dose accurately using the provided measuring spoon or cup.
Mechanism of Action
Desloratadine is a long-acting, non-sedating, peripheral histamine H1-receptor antagonist. It selectively blocks peripheral H1-receptors, thereby inhibiting the effects of histamine and alleviating allergic symptoms.
Pharmacokinetics
Onset
Within 1-3 hours.
Excretion
Primarily excreted in urine and feces as metabolites.
Half life
Approximately 27 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Cmax) reached in approximately 3 hours.
Metabolism
Extensively metabolized in the liver to 3-hydroxydesloratadine, an active metabolite, primarily via cytochrome P450 2D6 (CYP2D6).
Side Effects
Contraindications
- Hypersensitivity to desloratadine, loratadine, or any component of the formulation.
- Patients with a history of allergic reactions to other antihistamines (e.g., loratadine).
Drug Interactions
Alcohol
While desloratadine is non-sedating, concomitant use with alcohol may enhance central nervous system depression, so caution is advised.
Ketoconazole, Erythromycin, Azithromycin, Fluoxetine, Cimetidine
May increase plasma concentrations of desloratadine; however, no clinically significant adverse effects have been reported in clinical trials. Monitor for increased side effects if co-administered.
Storage
Store below 30°C in a dry place. Protect from direct light and moisture. Do not freeze.
Overdose
Symptoms of overdose may include somnolence, increased heart rate (tachycardia), and headache. In case of overdose, seek medical attention immediately. Management is symptomatic and supportive. Hemodialysis is not effective in removing desloratadine.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Desloratadine passes into breast milk, so caution should be exercised when administered to a nursing woman; a decision should be made whether to discontinue nursing or to discontinue the drug.

Clinical Trials
Extensive placebo-controlled and active-controlled clinical trials have demonstrated the efficacy and safety of desloratadine in treating symptoms of allergic rhinitis and chronic idiopathic urticaria across various patient populations, including children and adults.
Lab Monitoring
- No routine lab monitoring is generally required for desloratadine unless specific clinical indications arise (e.g., liver function tests in patients with hepatic impairment).
Doctor Notes
- Advise patients that while desloratadine is generally non-sedating, individual responses can vary, and caution is advised when performing tasks requiring mental alertness.
- For patients with severe renal or hepatic impairment, consider appropriate dose adjustment and monitoring.
- Emphasize consistent daily dosing for optimal symptom control.
Patient Guidelines
- Do not exceed the recommended dose; higher doses do not increase efficacy but may increase side effects.
- Inform your doctor or pharmacist if you have a history of liver or kidney problems.
- Report any new or worsening symptoms to your healthcare provider.
- Shake the bottle well before each use.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Desloratadine is generally considered non-sedating at recommended doses. However, individuals should assess their response to the medication before driving or operating machinery, especially when initiating therapy or in combination with other CNS depressants.
Lifestyle Advice
- Avoid known allergens that trigger your symptoms.
- Maintain good indoor air quality by regularly cleaning and using air purifiers if necessary.
- Stay hydrated and get adequate rest.
