Description
Overview of the medicine
A combination pain reliever containing Tramadol and Paracetamol, used for moderate to severe pain.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets every 4-6 hours as needed, not exceeding 8 tablets per day.
Elderly
Lower doses may be required, closely monitor for adverse effects.
Renal_impairment
Adjust dose and increase dosing interval (e.g., every 12 hours) for patients with creatinine clearance <30 mL/min.
How to Take
Oral. Can be taken with or without food. Swallow whole, do not crush or chew.
Mechanism of Action
Tramadol acts centrally by binding to mu-opioid receptors and inhibiting reuptake of norepinephrine and serotonin. Paracetamol inhibits prostaglandin synthesis in the central nervous system.

Absorption
Both Tramadol and Paracetamol are well absorbed orally. Tramadol's bioavailability is about 75%. Paracetamol's absorption is rapid and almost complete.
Metabolism
Both are metabolized in the liver. Tramadol undergoes O-demethylation (to active metabolite M1) and N-demethylation, followed by glucuronidation. Paracetamol undergoes glucuronidation and sulfation.
Side Effects
Contraindications
- Hypersensitivity to tramadol, paracetamol, or any component
- Acute alcohol, hypnotics, analgesics, opioids, psychotropic drugs intoxication
- Severe respiratory depression
- Concurrent use with MAOIs
- Uncontrolled epilepsy
Drug Interactions
MAOIs
Risk of serotonin syndrome and opioid toxicity.
Warfarin
Increased INR (International Normalized Ratio) and bleeding risk with Paracetamol.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
Carbamazepine
Decreased tramadol efficacy.
CNS Depressants (alcohol, sedatives, hypnotics)
Increased risk of respiratory depression and profound sedation.

Store below 30°C, in a cool and dry place, away from light and moisture. Keep out of reach of children.

Symptoms include respiratory depression, seizures, coma (tramadol) and hepatic necrosis (paracetamol). Management involves supportive care, naloxone for opioid effects, and N-acetylcysteine for paracetamol toxicity.
Pregnancy & Lactation
Not recommended during pregnancy unless clearly necessary. Avoid during lactation as both components pass into breast milk.
