Description
Overview of the medicine
Cefaclav is an oral antibiotic combining Cefixime, a third-generation cephalosporin, and Clavulanic Acid, a beta-lactamase inhibitor. It is used to treat various bacterial infections including respiratory tract, urinary tract, and skin infections.
Uses & Indications
Dosage
Adults
Cefixime 200 mg + Clavulanic Acid 125 mg, twice daily for 5-10 days, or as directed by physician.
Elderly
No specific dose adjustment typically required unless severe renal impairment is present. Monitor renal function.
Renal_impairment
Dosage adjustment necessary in severe renal impairment (CrCl < 30 mL/min). Consult physician.
How to Take
Take orally, with or without food. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Clavulanic Acid inactivates beta-lactamase enzymes produced by bacteria, which would otherwise degrade Cefixime, thereby extending its antibacterial spectrum.
Pharmacokinetics
Onset
Antibacterial effects typically begin within a few hours of administration.
Excretion
Cefixime is primarily excreted renally (about 50% unchanged in urine). Clavulanic acid is excreted renally and non-renally.
Half life
Cefixime: 3-4 hours; Clavulanic acid: approximately 1 hour.
Absorption
Cefixime is approximately 40-50% absorbed orally. Clavulanic acid is well absorbed.
Metabolism
Cefixime undergoes limited metabolism. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime, Clavulanic Acid, other cephalosporins, or penicillins.
- History of severe allergic reaction to beta-lactam antibiotics.
Drug Interactions
Warfarin
May potentiate anticoagulant effect, increasing risk of bleeding. Monitor INR.
Probenecid
May increase Cefixime plasma levels.
Carbamazepine
Increased carbamazepine levels reported with concomitant Cefixime.
Oral Contraceptives
Reduced efficacy of oral contraceptives (rarely reported).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include nausea, vomiting, diarrhea, and central nervous system disturbances. Treatment is symptomatic and supportive. Hemodialysis or peritoneal dialysis is not typically effective.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy and lactation; consult a doctor to weigh benefits against potential risks.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of Cefixime and Clavulanic Acid combination for various bacterial infections.
Lab Monitoring
- Monitor renal function (creatinine clearance) in patients with pre-existing renal impairment.
- Periodic complete blood counts (CBC) during prolonged therapy.
Doctor Notes
- Consider culture and susceptibility testing before initiating treatment if appropriate.
- Educate patients on the importance of completing the full antibiotic course.
- Be aware of potential drug interactions, especially with anticoagulants.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if you feel better, to prevent bacterial resistance.
- Do not save medicine for future infections.
- Inform your doctor about any pre-existing medical conditions or other medications you are taking.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
This medication may cause dizziness in some individuals. If affected, avoid driving or operating machinery.
Lifestyle Advice
