Overview of the medicine
Terlipressin (Varipress) is a synthetic analogue of vasopressin primarily used for the treatment of acute bleeding from esophageal varices and Type 1 Hepatorenal Syndrome (HRS). It works by causing vasoconstriction in the splanchnic circulation, thereby reducing portal pressure and stopping bleeding.

Dosage
Adults
Bleeding Esophageal Varices: Initial dose 2 mg IV bolus over 3 minutes, followed by 1-2 mg IV every 4-6 hours. Maximum daily dose 12 mg. Hepatorenal Syndrome: Initial dose 0.85 mg IV bolus every 6 hours for 3 days, increasing to 1.7 mg every 6 hours if serum creatinine doesn't decrease.
Elderly
Use with caution due to increased risk of cardiovascular adverse events. No specific dosage adjustment recommended, but closer monitoring is advised.
Renal_impairment

How to Take

Mechanism of Action
Terlipressin is a prodrug that is slowly cleaved by endopeptidases to release its active metabolite, lysine-vasopressin. This reduces portal venous inflow and pressure, making it effective in controlling variceal bleeding. It also improves systemic hemodynamics in Hepatorenal Syndrome.
