Veelac 335 gm Oral Solution is a synthetic disaccharide used as an osmotic laxative to treat chronic constipation and hepatic encephalopathy. It helps soften stools and promote bowel movements by drawing water into the colon. In hepatic encephalopathy, it reduces blood ammonia levels.
Uses & Indications
Dosage
Adults
For constipation: Initially 15-45 mL daily; maintenance 15-30 mL daily. For hepatic encephalopathy: Initially 30-45 mL 3-4 times daily, adjusted to produce 2-3 soft stools per day.
Elderly
Same as adults, but may require lower initial doses and slower titration. Use with caution in patients with underlying comorbidities.
Renal_impairment
No dose adjustment required as it is minimally absorbed and renally excreted.
How to Take
Can be taken orally, either undiluted or diluted with water, fruit juice, or milk. For constipation, usually taken as a single daily dose or divided into two doses. For hepatic encephalopathy, doses are typically divided throughout the day.
Mechanism of Action
Lactulose is a disaccharide that is not absorbed in the small intestine. It passes unchanged into the large intestine, where it is metabolized by colonic bacteria into low molecular weight organic acids (lactic acid, acetic acid). These acids lower the pH of the colonic contents and increase the osmotic pressure, leading to an influx of water into the colon. This softens the stool and stimulates peristalsis, promoting defecation. In hepatic encephalopathy, the acidification of the colonic contents traps ammonia as ammonium ions, preventing its absorption into the bloodstream and facilitating its excretion.

Constipation: 24-48 hours; Hepatic Encephalopathy: 24 hours to 3 days.
Excretion
Unabsorbed lactulose and its metabolites are excreted primarily in feces. Absorbed portion (minimal) is excreted in urine.
