Vegara 50 mg Tablet contains Sildenafil Citrate, a medication primarily used to treat erectile dysfunction (ED) in men. It works by increasing blood flow to the penis during sexual stimulation. It may also be used in some cases for pulmonary arterial hypertension.
Uses & Indications
Dosage
Adults
For Erectile Dysfunction: The recommended dose is 50 mg taken approximately one hour before sexual activity. Based on efficacy and toleration, the dose may be increased to 100 mg or decreased to 25 mg. The maximum recommended dose is 100 mg once per day. For Pulmonary Arterial Hypertension: 20 mg three times daily.
Elderly
A starting dose of 25 mg should be considered in elderly patients (≥ 65 years) due to age-related decrease in clearance.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take Vegara 50 mg Tablet orally, with or without food, approximately one hour before sexual activity. Do not take more than one dose per day.
Mechanism of Action
Sildenafil is a selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). When sexual stimulation causes local release of nitric oxide, PDE5 inhibition by sildenafil increases cGMP levels in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood into the penis.

Excretion
Excreted predominantly as metabolites in the feces (approximately 80% of the administered oral dose) and to a lesser extent in the urine (approximately 13% of the administered oral dose).

Rapidly absorbed after oral administration. Mean absolute bioavailability is about 41%. Peak plasma concentrations are reached within 30 to 120 minutes (median 60 minutes).
Metabolism
Primarily metabolized by hepatic CYP3A4 (major route) and CYP2C9 (minor route) microsomal isoenzymes.
Side Effects
Contraindications
- Concomitant use of nitrates in any form (e.g., nitroglycerin, isosorbide dinitrate) due to potentiation of hypotensive effects.
- Hypersensitivity to sildenafil or any component of the tablet.
- Concomitant use with riociguat.
- Patients with severe cardiovascular events (e.g., myocardial infarction, stroke) in recent history.
- Hereditary degenerative retinal disorders, like retinitis pigmentosa.
