Description
Overview of the medicine
Venlin 4 mg Tablet contains Carvedilol, a beta-blocker with additional alpha1-blocking properties. It is used to treat high blood pressure, stable chronic angina, and symptomatic chronic heart failure.
Uses & Indications
Dosage
Adults
Hypertension: Initially 6.25 mg twice daily, may be increased to 12.5-25 mg twice daily. Chronic Heart Failure: Initially 3.125 mg twice daily, slowly titrated up to a maximum of 25 mg twice daily (for patients <85 kg) or 50 mg twice daily (for patients >85 kg). The 4 mg strength is typically used in titration schedules.
Elderly
No specific dose adjustment for elderly patients, but use with caution and monitor closely.
Renal_impairment
No dosage adjustment usually required for mild to moderate renal impairment; caution in severe impairment.
How to Take
Take orally, preferably with food, to minimize the risk of orthostatic hypotension.
Mechanism of Action
Carvedilol blocks beta1, beta2, and alpha1 adrenergic receptors. Beta-blockade reduces heart rate and contractility, while alpha1-blockade causes vasodilation, leading to reduced peripheral vascular resistance and blood pressure.
Pharmacokinetics
Onset
Within 1 hour for blood pressure reduction.
Excretion
Primarily via bile into feces; a small portion via kidneys.
Half life
Approximately 7-10 hours.
Absorption
Rapid and extensive absorption, but significant first-pass metabolism leads to approximately 25-35% absolute bioavailability. Food delays absorption but increases bioavailability.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP2C9 enzymes.
Side Effects
Contraindications
- •Decompensated heart failure requiring intravenous inotropic therapy
- •Bronchial asthma or related bronchospastic conditions
- •Second or third-degree AV block (unless permanent pacemaker present)
- •Sick sinus syndrome (unless permanent pacemaker present)
- •Severe bradycardia (<50 bpm)
- •Cardiogenic shock
- •Severe hepatic impairment
- •Hypersensitivity to Carvedilol or any component of the formulation
Drug Interactions
Digoxin
May increase digoxin levels; monitor digoxin concentration.
Cimetidine
May increase carvedilol bioavailability.
Rifampicin
May decrease carvedilol plasma levels.
Insulin or oral hypoglycemics
May enhance the blood glucose-lowering effect and mask symptoms of hypoglycemia.
Alpha-1 blockers (e.g., Prazosin)
Increased risk of orthostatic hypotension.
Calcium channel blockers (e.g., Verapamil, Diltiazem)
Additive effects on AV conduction or myocardial contractility; monitor ECG and blood pressure.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypotension, bradycardia, cardiac insufficiency, cardiogenic shock, and cardiac arrest. Management involves symptomatic and supportive treatment, including IV fluids, vasopressors, atropine, and glucagon as needed.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Carvedilol is excreted in human breast milk; therefore, breastfeeding is not recommended.
