Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the maintenance treatment of asthma and to relieve symptoms of seasonal allergies.
Uses & Indications
Dosage
Adults
10 mg once daily, in the evening for asthma or as needed for allergic rhinitis.
Elderly
No dosage adjustment generally needed.
Renal_impairment
No dosage adjustment needed.
How to Take
Oral. Take once daily in the evening for asthma. For allergic rhinitis, take once daily, timing depends on patient needs. Can be taken with or without food.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, thereby inhibiting the actions of cysteinyl leukotrienes (LTC4, LTD4, LTE4). These leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils, and are implicated in asthma and allergic rhinitis.
Pharmacokinetics
Onset
Clinical improvement usually within 1 day for asthma; within 1 day for allergic rhinitis.
Excretion
Primarily excreted in feces via bile (approximately 86%), with minimal renal excretion (less than 0.2%).
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are achieved in approximately 2-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2C8, CYP3A4, and CYP2C9.

- Hypersensitivity to Montelukast or any component of the tablet

Rifampin
May decrease montelukast plasma levels.
Phenobarbital
May decrease montelukast plasma levels.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed during pregnancy. Excreted in breast milk, caution advised during lactation.
