Description
Overview of the medicine
Mifepristone 200 mg is an anti-progesterone medication used for medical termination of early pregnancy (up to 63 days of gestation) in combination with misoprostol. It blocks the effects of progesterone, which is necessary for maintaining pregnancy.
Uses & Indications
Dosage
Adults
200 mg orally as a single dose, followed by Misoprostol 800 mcg orally or vaginally 24-48 hours later.
Elderly
No specific dosage adjustment required, but caution is advised due to potential comorbidities.
Renal_impairment
No specific guidelines; caution and monitoring are recommended.
How to Take
Oral administration with water. It should be taken under medical supervision.
Mechanism of Action
It is a synthetic steroid that competitively binds to progesterone receptors, leading to the detachment of the conceptus from the uterine wall and softening of the cervix. It also sensitizes the myometrium to prostaglandin-induced contractions.
Pharmacokinetics
Onset
Uterine contractions and cervical changes typically begin within 24-48 hours after administration.
Excretion
Primarily through feces (83%), with a smaller portion excreted in urine (9%).
Half life
Approximately 18-36 hours for the parent compound, with active metabolites having a longer half-life.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1.5 hours. Bioavailability is about 69%.
Metabolism
Primarily hepatic (liver) metabolism via CYP3A4, with extensive first-pass effect.
