Overview of the medicine
Verifen-SR 100 mg Capsule contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain, inflammation, and fever. The sustained-release formulation provides prolonged therapeutic effects.
Uses & Indications
Dosage
Adults
Usually 100 mg once daily. Max 100 mg/day for sustained release formulation.
Elderly
Lower doses may be required, close monitoring for adverse effects is recommended.
Renal_impairment
Use with caution; dose adjustment may be necessary in severe impairment. Not recommended for severe renal failure.
How to Take
Take the capsule whole with a glass of water, preferably after a meal to reduce gastrointestinal upset. Do not chew or crush.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, leading to a reduction in the synthesis of prostaglandins. Prostaglandins are mediators of pain, inflammation, and fever, so their reduction results in analgesic, anti-inflammatory, and antipyretic effects.

- History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs
- Active peptic ulcer or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery (peri-operative pain)
Drug Interactions
SSRIs
Increased risk of gastrointestinal bleeding.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
Lithium, Digoxin, Methotrexate
Increased plasma levels of these drugs.
Warfarin, other anticoagulants
Increased risk of bleeding.
Diuretics, ACE inhibitors, Angiotensin II Receptor Blockers (ARBs)
Reduced antihypertensive effect; increased risk of renal impairment.

Symptoms include lethargy, drowsiness, nausea, vomiting, epigastric pain, GI bleeding, hypotension, respiratory depression, and coma. Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Category C in early pregnancy; contraindicated in the third trimester due to risk of premature closure of the fetal ductus arteriosus. Not recommended during breastfeeding as it is excreted in breast milk.
