Overview of the medicine
Sildenafil Citrate is used for erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). It increases blood flow to the penis to help achieve and maintain an erection.
Uses & Indications
Dosage
Adults
For ED, typical starting dose is 50 mg taken approximately 1 hour before sexual activity. May be adjusted to 25 mg or 100 mg based on efficacy and tolerability. Max one dose per day.
Elderly
Initial dose of 25 mg may be considered.
Renal_impairment
Initial dose of 25 mg for severe renal impairment (CrCl < 30 mL/min).
How to Take
Orally, with or without food, approximately 30-60 minutes before sexual activity.
Mechanism of Action
Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5), which is responsible for the degradation of cGMP in the corpus cavernosum.

Excretion
Primarily excreted as metabolites in the feces (approximately 80%) and to a lesser extent in the urine (approximately 13%).

Approximately 3-5 hours.
Absorption
Rapidly absorbed. Absolute bioavailability is approximately 41%. Peak plasma concentrations are reached within 30-120 minutes (median 60 minutes).
Metabolism
Primarily metabolized by cytochrome P450 (CYP3A4 major route, CYP2C9 minor route) in the liver.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin)
- Hypersensitivity to sildenafil
- Concomitant use with guanylate cyclase stimulators (e.g., riociguat)
- Recent stroke or myocardial infarction
- Severe hepatic impairment
