Description
Overview of the medicine
Ivermectin is an antiparasitic medication used to treat certain parasitic infections, including onchocerciasis, strongyloidiasis, and scabies. It works by paralyzing and killing parasites.
Uses & Indications
Dosage
Adults
Dosage varies by indication. For Strongyloidiasis, a single oral dose of 200 mcg/kg body weight. For Onchocerciasis, a single oral dose of 150 mcg/kg body weight, repeated at intervals of 3, 6, or 12 months as needed. For Scabies, a single oral dose of 200 mcg/kg body weight, with a second dose 7-14 days later if necessary.
Elderly
Similar to adult dosage; however, caution should be exercised in elderly patients due to potential decreased renal, hepatic, or cardiac function.
Renal_impairment
No specific dosage adjustment is generally required for mild to moderate renal impairment, but caution is advised in severe cases due to limited data.
How to Take
Take orally with a glass of water, usually on an empty stomach (e.g., 1 hour before a meal). Follow your doctor's instructions precisely.
Mechanism of Action
Ivermectin selectively binds with high affinity to glutamate-gated chloride ion channels which are present in invertebrate nerve and muscle cells. This binding leads to an increase in the permeability of the cell membrane to chloride ions, resulting in hyperpolarization of the nerve or muscle cell, leading to paralysis and death of the parasite.
Pharmacokinetics
Onset
Clinical effects may be seen within hours to days, depending on the parasitic infection.
Excretion
Excreted almost exclusively in the feces as unchanged drug and metabolites; very little is excreted in urine.
Half life
Approximately 12-56 hours.
Absorption
Well absorbed after oral administration; bioavailability is approximately 28%.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Ivermectin or any component of the tablet.
- Pregnancy and lactation (relative contraindication, use only if potential benefit justifies the potential risk).
- Children weighing less than 15 kg (or as per specific guidelines).
Drug Interactions
Warfarin
May enhance the anticoagulant effect, requiring closer monitoring of INR/PT.
Other CNS depressants
May potentiate sedative effects.
CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin)
May increase Ivermectin plasma concentrations, increasing risk of side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include rash, edema, headache, dizziness, nausea, vomiting, diarrhea, abdominal pain, seizures, ataxia, dyspnea, and coma. Treatment is symptomatic and supportive, including gastric lavage and supportive therapy as needed.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, caution should be exercised when administered to a nursing woman.

- Ophthalmic examination (particularly important in Onchocerciasis treatment to monitor for Mazzotti reaction).
Doctor Notes
- Ensure accurate diagnosis of parasitic infection before prescribing.
- Calculate dose based on patient's body weight.
- Advise patients on potential Mazzotti reaction when treating Onchocerciasis.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking the medicine early, even if you feel better.
- Report any unusual or severe side effects to your doctor immediately.
