Description
Overview of the medicine
Solifenacin succinate is an antimuscarinic agent used to treat symptoms of overactive bladder. It helps relax the bladder muscles, reducing urgency, frequency, and incontinence.
Uses & Indications
Dosage
Adults
Initially 5 mg once daily, may be increased to 10 mg once daily based on efficacy and tolerability.
Elderly
No dosage adjustment required.
Renal_impairment
Max 5 mg daily for severe renal impairment (CrCl < 30 mL/min).
How to Take
Swallow the tablet whole with liquid, with or without food. Do not crush or chew.
Mechanism of Action
Solifenacin is a competitive muscarinic receptor antagonist. It works by blocking muscarinic receptors (predominantly M3 subtype) in the bladder, which are responsible for bladder muscle contraction. This inhibition leads to relaxation of the detrusor muscle, increasing bladder capacity and decreasing the frequency of involuntary contractions.
Pharmacokinetics
Onset
Effects seen within a few days, full effect in about 2-4 weeks.
Excretion
Primarily excreted in urine (approximately 69%), with a smaller portion in feces (approximately 23%).
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached 3-8 hours after administration. Absolute bioavailability is approximately 90%.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4. Major metabolite is 4R-hydroxysolifenacin, which is active.
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Myasthenia gravis
- Severe hepatic impairment
- Hypersensitivity to solifenacin succinate or any component of the product.
Drug Interactions
Cholinergic agonists
May antagonize the effects of solifenacin.
Drugs that prolong QT interval
Use with caution.
CYP3A4 inducers (e.g., Rifampicin, Carbamazepine)
May decrease solifenacin exposure.
CYP3A4 inhibitors (e.g., Ketoconazole, Ritonavir)
Concomitant use with strong CYP3A4 inhibitors may increase solifenacin exposure; limit solifenacin to 5 mg daily.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe anticholinergic effects (e.g., severe dry mouth, blurred vision, urinary retention, tachycardia). Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Unknown if excreted in human milk; exercise caution when administering to nursing mothers.

Clinical Trials
Extensive clinical trials have demonstrated Solifenacin's efficacy and safety in treating overactive bladder symptoms, showing significant reductions in urinary frequency, urgency, and incontinence episodes compared to placebo.
Lab Monitoring
- No specific routine lab monitoring is required. Liver and renal function should be assessed in patients with hepatic or renal impairment.
Doctor Notes
- Counsel patients on potential anticholinergic side effects, particularly dry mouth and constipation, and strategies for management.
- Assess renal and hepatic function before initiating and periodically during treatment, especially in susceptible populations.
- Advise patients to report any signs of urinary retention or severe abdominal pain promptly.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking the medicine without consulting your doctor, even if you feel better.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
