Description
Overview of the medicine
Vesocal 10 mg Tablet contains Solifenacin Succinate, an anticholinergic medication used to treat symptoms of overactive bladder, such as frequent urination, urgency, and incontinence. It works by relaxing the bladder muscles.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily. May be increased to 10 mg once daily based on efficacy and tolerability.
Elderly
No dose adjustment generally required, but caution is advised.
Renal_impairment
Mild to moderate (CrCl > 30 mL/min): Maximum 10 mg/day. Severe (CrCl ≤ 30 mL/min): Maximum 5 mg/day.
How to Take
Vesocal tablets should be taken orally once daily with water, with or without food. They should be swallowed whole and not chewed or crushed.
Mechanism of Action
Solifenacin is a competitive muscarinic receptor antagonist. It specifically binds to M3 muscarinic receptors on the detrusor muscle of the bladder, inhibiting acetylcholine's action. This reduces involuntary contractions of the bladder muscle, thereby increasing bladder capacity and decreasing the frequency of urination, urgency, and incontinence episodes.
Pharmacokinetics
Onset
Clinical effects generally observed within 1 week, maximal effect within 2-8 weeks.
Excretion
Primarily excreted in the urine (approximately 70%) and feces (approximately 23%) as metabolites and some unchanged drug.
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 90%. Peak plasma concentrations are reached 3-8 hours after administration.
Metabolism
Extensively metabolized by the liver, primarily by CYP3A4, to several inactive metabolites.
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Severe liver impairment
- Severe renal impairment (CrCl < 10 mL/min)
- Patients on strong CYP3A4 inhibitors (e.g., ketoconazole) with moderate to severe renal or hepatic impairment
- Known hypersensitivity to Solifenacin or any excipients
Drug Interactions
Other Anticholinergics
Additive anticholinergic effects.
Cholinergic Receptor Agonists
May reduce the effect of cholinergic medications.
Drugs that prolong QT interval
Caution advised.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Increased Solifenacin exposure. Dose adjustment may be needed.
Prokinetic agents (e.g., Metoclopramide, Cisapride)
Solifenacin may decrease their effect.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe anticholinergic effects such as blurred vision, dry mouth, urinary retention, constipation, CNS excitation (e.g., hallucinations, severe drowsiness), and tachycardia. Treatment is supportive and symptomatic. Gastric lavage and activated charcoal may be considered. Physostigmine may be used for severe central anticholinergic effects.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Solifenacin is excreted in human milk. Caution should be exercised when administered to a nursing woman.
