Description
Overview of the medicine
Solifenacin succinate is an antimuscarinic agent used to treat symptoms of overactive bladder, such as urinary incontinence, urgency, and frequency. It helps to reduce involuntary bladder contractions.
Uses & Indications
Dosage
Adults
The recommended starting dose is 5 mg once daily. If needed, the dose may be increased to 10 mg once daily.
Elderly
No dosage adjustment is generally required, but caution should be exercised.
Renal_impairment
For patients with severe renal impairment (creatinine clearance < 30 mL/min), the maximum dose should be 5 mg once daily.
How to Take
Take orally with or without food. Swallow the tablet whole with water; do not chew, crush, or break it.
Mechanism of Action
Solifenacin is a competitive muscarinic receptor antagonist. It selectively blocks M3 muscarinic receptors in the bladder, which leads to the relaxation of the detrusor muscle, increasing bladder capacity and reducing involuntary bladder contractions.
Pharmacokinetics
Onset
Initial effects may be seen within a few weeks, with full therapeutic effect typically achieved within 4-8 weeks.
Excretion
Primarily excreted in urine (approximately 70%) and feces (approximately 20%) after hepatic metabolism.
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentration reached in 3-8 hours. Bioavailability is approximately 90%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 3A4 (CYP3A4) enzyme system.
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Severe hepatic impairment
- Hypersensitivity to the active substance or to any of the excipients
Drug Interactions
Anticholinergic agents
Concomitant use may potentiate anticholinergic effects and side effects. Avoid excessive use.
Cholinergic receptor agonists
May reduce the effect of solifenacin.
Drugs that prolong QT interval
Use with caution due to theoretical risk of QT prolongation, although not clinically significant with solifenacin alone.
CYP3A4 inducers (e.g., rifampicin, phenytoin)
May decrease solifenacin plasma concentrations.
Potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
Can significantly increase solifenacin exposure. The maximum dose of solifenacin should be 5 mg/day.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe anticholinergic effects like severe dry mouth, blurred vision, urinary retention, tachycardia, and hallucinations. Treatment is symptomatic and supportive. Gastric lavage, activated charcoal, and in severe cases, physostigmine may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known whether solifenacin is excreted in human milk; caution should be exercised when administered to a nursing mother.

Clinical Trials
Solifenacin has been extensively studied in various clinical trials demonstrating its efficacy and safety in patients with overactive bladder symptoms. Studies have shown significant improvements in urinary frequency, urgency, and incontinence episodes.
Lab Monitoring
- No specific routine lab monitoring is required. Liver and kidney function tests may be considered in patients with pre-existing impairment.
Doctor Notes
- Educate patients on proper administration and potential side effects, particularly anticholinergic effects like dry mouth and constipation.
- Assess for urinary retention and signs of bladder outflow obstruction before initiating treatment and during follow-up.
- Consider dose reduction in patients with severe renal impairment or moderate hepatic impairment, or when co-administered with potent CYP3A4 inhibitors.
- Advise patients that full therapeutic benefits may not be apparent for several weeks.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking the medicine without consulting your doctor.
- Report any new or worsening side effects immediately.
- Be aware that the full effect of the medicine may take several weeks to develop.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Vesocal-plus may cause blurred vision, dizziness, or somnolence. Patients should be cautious when driving or operating machinery until they know how the medicine affects them.
Lifestyle Advice
- Avoid excessive consumption of caffeine and alcohol, as they can irritate the bladder and worsen OAB symptoms.
- Maintain a healthy fluid intake throughout the day, but avoid large amounts right before bedtime.
- Practice bladder training techniques, if advised by your doctor.
