Overview of the medicine
Vesol-N 0.05% Cream is a topical preparation containing Clobetasol Propionate, a highly potent corticosteroid, and Neomycin Sulfate, an aminoglycoside antibiotic. It is used to treat various inflammatory skin conditions that are complicated by secondary bacterial infections.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area twice daily, or as directed by the physician. Treatment should be limited to 2 weeks continuously and total amount applied should not exceed 50g per week.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential thinning skin and increased absorption.
Renal_impairment
Use with caution due to potential systemic absorption of Neomycin, especially if skin integrity is compromised. Consult physician.
How to Take
For external use only. Apply a thin layer to the affected area and gently rub it in. Wash hands before and after application. Avoid contact with eyes, mouth, and other mucous membranes. Do not use on the face, groin, or axillae unless directed by a physician. Do not use with occlusive dressings unless specifically instructed.
Mechanism of Action
Clobetasol Propionate exerts anti-inflammatory, antipruritic, and vasoconstrictive effects by inducing phospholipase A2 inhibitory proteins, lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes. Neomycin Sulfate is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by irreversibly binding to the 30S ribosomal subunit, leading to bacterial cell death.

Topical effects usually seen within days.
Excretion
Clobetasol and its metabolites are excreted primarily by the kidneys, with some bile excretion. Unabsorbed Neomycin is excreted in feces.
Half life
Systemic half-life of Clobetasol is approximately 3-4 hours; Neomycin is variable with minimal systemic absorption.
Absorption
Systemic absorption of Clobetasol Propionate varies depending on the integrity of the epidermal barrier and the use of occlusive dressings. It can be absorbed systemically. Neomycin Sulfate is poorly absorbed through intact skin.
Metabolism
Clobetasol is metabolized primarily in the liver. Neomycin is minimally metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Clobetasol Propionate, Neomycin Sulfate, or any other components of the cream.
- •Rosacea, acne vulgaris, perioral dermatitis.
- •Untreated fungal, viral, or primary bacterial skin infections.
- •Pruritus without inflammation.
- •Perianal and genital pruritus.
Drug Interactions
Corticosteroids (systemic)
Concurrent use may increase systemic corticosteroid effects and HPA axis suppression.
Other topical preparations
Avoid concurrent use of other topical steroids or neomycin-containing products to prevent additive effects or resistance.
Drugs metabolized by CYP3A4 (e.g., Ritonavir, Itraconazole)
May inhibit metabolism of Clobetasol, increasing systemic exposure.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause acute problems. However, chronic overdose or misuse can lead to systemic effects of hypercorticism (e.g., Cushing's syndrome) and, with neomycin, potential ototoxicity or nephrotoxicity with significant absorption. Treatment is symptomatic and supportive. Gradual withdrawal of the steroid may be necessary.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Avoid prolonged use or large quantities during pregnancy. It is unknown whether topically administered corticosteroids are excreted in human milk. Use with caution during lactation. Neomycin is poorly absorbed topically, but caution is advised.
