Description
Overview of the medicine
Trimetazidine is a metabolic agent used to prevent angina attacks and treat other conditions like vertigo and tinnitus. The modified-release formulation allows for once or twice daily dosing.
Uses & Indications
Dosage
Adults
1 tablet (35 mg) twice daily, morning and evening, during meals.
Elderly
Dosage adjustment may not be required. However, caution is advised in patients with moderate renal impairment.
Renal_impairment
For patients with moderate renal impairment (creatinine clearance 30-60 mL/min), the recommended dose is 1 tablet (35 mg) once daily, in the morning, during breakfast.
How to Take
To be taken orally, with meals. Swallow the tablet whole with a glass of water, do not chew or crush.
Mechanism of Action
Trimetazidine preserves myocardial energy metabolism during ischemia. It does this by shifting the myocardial glucose oxidation from fatty acid oxidation, thereby enhancing glucose utilization and improving ATP production.

Onset
Clinical effects typically observed within 2 weeks of regular use.
Excretion
Primarily eliminated unchanged in the urine.
Half life
Approximately 4-6 hours for the standard release, but prolonged for modified-release formulations (around 7 hours for 35mg MR).
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentration reached within 2-3 hours.
Metabolism
Minimal hepatic metabolism.
