Overview of the medicine
Vgofil 10 mg Tablet contains Vasodilafil, a potent phosphodiesterase-5 (PDE5) inhibitor primarily used to treat erectile dysfunction and symptoms of benign prostatic hyperplasia.
Uses & Indications
Dosage
Adults
For ED: 10 mg once daily, prior to anticipated sexual activity. For BPH: 5 mg once daily. Max single dose 20 mg. Max frequency: once daily.
Elderly
No dose adjustment generally required, but caution is advised in very elderly patients (≥75 years) due to potential for increased exposure.
Renal_impairment
For severe renal impairment (creatinine clearance <30 mL/min): Initial dose of 5 mg every other day. Max dose 10 mg every 48 hours. For mild to moderate: No adjustment needed.
How to Take
Take orally with or without food. Do not split, crush, or chew the tablet. It should be swallowed whole.
Mechanism of Action
Vasodilafil inhibits phosphodiesterase type 5 (PDE5), an enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP). By inhibiting PDE5, Vasodilafil increases cGMP levels, leading to smooth muscle relaxation and increased blood flow in certain areas, thereby facilitating erection and improving urinary symptoms.
Pharmacokinetics
Onset
Onset of action for ED is typically within 30-60 minutes; for BPH symptoms, improvement is gradual over several days.
Excretion
Mainly excreted in feces (approximately 61%) and to a lesser extent in urine (approximately 36%).
Half life
Approximately 17.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically reached within 30-120 minutes.
Metabolism
Primarily metabolized by cytochrome P450 (CYP) isoenzymes, mainly CYP3A4, in the liver.
Side Effects
Contraindications
- •Concomitant use with nitrates or nitric oxide donors
- •Hypersensitivity to Vasodilafil or any component of the tablet
- •Patients with severe cardiovascular disease for whom sexual activity is inadvisable
Drug Interactions
Nitrates
Concomitant use is contraindicated due to the risk of severe hypotension.
Alpha-blockers
May cause symptomatic hypotension; dose adjustment of either drug may be needed.
Antihypertensives
May result in additive blood pressure lowering effects.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May increase Vasodilafil exposure; dose adjustment recommended.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, standard supportive measures should be adopted as required. Haemodialysis is not expected to accelerate clearance as Vasodilafil is highly protein bound.
Pregnancy & Lactation
Vgofil is not indicated for use in women. No data are available on its use in pregnant or lactating women.
