Overview of the medicine
Sildenafil is a medication used to treat erectile dysfunction (ED). It helps men achieve and maintain an erection during sexual stimulation by increasing blood flow to the penis.
Uses & Indications
Dosage
Adults
The recommended dose is 50 mg taken approximately 1 hour before sexual activity. The dose may be increased to 100 mg or decreased to 25 mg, based on efficacy and tolerability. Maximum recommended dose is 100 mg once daily.
Elderly
No dosage adjustment required for elderly patients.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take Vigor-100 mg tablet orally with water, approximately 30-60 minutes before sexual activity. It can be taken with or without food, but a high-fat meal may delay the onset of action.
Mechanism of Action
Sildenafil inhibits phosphodiesterase-5 (PDE5), an enzyme responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. By inhibiting PDE5, sildenafil increases cGMP levels, leading to smooth muscle relaxation and increased blood flow into the penis, resulting in an erection during sexual stimulation.

Excretion
Excreted predominantly in the feces (approximately 80% of administered oral dose) and to a lesser extent in the urine (approximately 13% of administered oral dose).
Half life
Approximately 3-5 hours.
Absorption
Rapidly absorbed. Absolute bioavailability is about 41%. Peak plasma concentrations are reached within 30-120 minutes (median 60 minutes) of oral dosing in the fasted state.
Metabolism
Primarily metabolized by cytochrome P450 (CYP3A4 and CYP2C9) in the liver. The major circulating metabolite, N-desmethyl sildenafil, has about 50% of the activity of the parent drug.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin) in any form, as it may potentiate hypotensive effects.
- Hypersensitivity to Sildenafil or any component of the tablet.
- Patients for whom sexual activity is inadvisable due to underlying cardiovascular conditions.
- Recent stroke or myocardial infarction.
- Known hereditary degenerative retinal disorders, like retinitis pigmentosa.
Drug Interactions
Nitrates
Potentiation of hypotensive effects, potentially life-threatening. Absolutely contraindicated.
Riociguat
Concomitant use is contraindicated due to risk of severe hypotension.
Alpha-blockers
May lead to symptomatic hypotension (low blood pressure), especially with initial doses of sildenafil. Start sildenafil at a lower dose if taking alpha-blockers.
CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir, erythromycin)
Can increase sildenafil plasma levels, necessitating a dose reduction of sildenafil.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly protein-bound and not significantly eliminated in the urine.
