Description
Overview of the medicine
Vilaro 25 mcg Inhalation Capsule contains Indacaterol, a long-acting beta2-adrenergic agonist (LABA), used for the maintenance treatment of chronic obstructive pulmonary disease (COPD) in adults. It helps to relax the muscles around the airways in the lungs, making it easier to breathe.
Uses & Indications
Dosage
Adults
One 75 mcg capsule once daily, or 25 mcg (for maintenance) once daily, inhaled using the specific inhaler device. Always follow your doctor's instructions.
Elderly
No dose adjustment is required in elderly patients.
Renal_impairment
No dose adjustment is required in patients with renal impairment.
How to Take
For oral inhalation only. The capsule should be placed into a specific inhaler device (e.g., Breezhaler or similar) and the powder inhaled through the mouth. Do not swallow the capsule.
Mechanism of Action
Indacaterol is a potent and ultra-long-acting beta2-adrenergic agonist. It stimulates beta2-adrenergic receptors in the smooth muscle of the bronchi, leading to bronchodilation. This relaxes the airway muscles, helping to open up the airways and improve airflow.
Pharmacokinetics
Onset
Within 5 minutes
Excretion
Mainly via feces (94%), with a small amount excreted in urine (less than 2%).
Half life
Approximately 40-56 hours
Absorption
Rapidly absorbed after oral inhalation, reaching peak plasma concentrations within 15-30 minutes.
Metabolism
Primarily metabolized by UGT1A1 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to indacaterol or any component of the formulation.
- Acute bronchospasm (not indicated for rescue therapy).
Drug Interactions
Beta-blockers
May reduce the effect of indacaterol and cause severe bronchospasm in COPD patients.
Other sympathomimetics
May potentiate the adverse effects of indacaterol.
Diuretics (loop or thiazide)
May potentiate hypokalemia.
Tricyclic antidepressants, MAO inhibitors
May potentiate the effect of indacaterol on the cardiovascular system.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase indacaterol exposure.
Storage
Store in a cool, dry place, below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, palpitations, headache, nausea, and vomiting. Treatment is symptomatic and supportive. Cardiac monitoring may be necessary.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if indacaterol is excreted in human milk; caution should be exercised when administered to a nursing woman.
