Description
Overview of the medicine
Vironil-A is a brand of Acyclovir, an antiviral drug used to treat infections caused by herpes simplex virus (HSV), varicella-zoster virus (VZV), and herpes zoster (shingles). It helps reduce the severity and duration of outbreaks.
Uses & Indications
Dosage
Adults
Genital herpes (initial): 200 mg 5 times daily for 10 days. Recurrent: 200 mg 5 times daily for 5 days or 400 mg 3 times daily for 5 days. Suppression: 400 mg twice daily. Herpes zoster: 800 mg 5 times daily for 7-10 days. Varicella: 800 mg 4 times daily for 5 days. Topical cream: Apply 5 times daily for 4 days.
Elderly
Consider reduced dosage in elderly patients with impaired renal function. Maintain adequate hydration.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For severe impairment, reduce dose and/or frequency (e.g., 200 mg twice daily for HSV, 800 mg twice daily for VZV).
How to Take
Oral tablets can be taken with or without food. Topical cream should be applied to affected areas, avoiding eyes. Intravenous form is for severe infections and should be administered by a healthcare professional.
Mechanism of Action
Acyclovir is a synthetic purine nucleoside analogue with in vitro and in vivo inhibitory activity against human herpes viruses. It is converted to its active triphosphate form by viral thymidine kinase, which then inhibits viral DNA synthesis and replication.
Pharmacokinetics
Onset
Variable, depending on indication and route of administration (e.g., within 24-48 hours for oral therapy for acute outbreaks).
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 2.5-3 hours in adults with normal renal function.
Absorption
Poor oral absorption (15-30% bioavailability). Food does not significantly affect absorption.
Metabolism
Minor hepatic metabolism; primarily converted to 9-carboxymethoxymethylguanine.
Side Effects
Contraindications
- •Hypersensitivity to acyclovir or valacyclovir.
- •Patients with severe renal impairment (relative contraindication for standard doses).
Drug Interactions
Cimetidine
May increase the AUC of acyclovir and decrease its renal clearance.
Probenecid
Increases acyclovir plasma concentrations and prolongs its half-life by reducing renal excretion.
Nephrotoxic drugs
Concomitant use with other nephrotoxic agents may increase the risk of renal dysfunction.
Mycophenolate Mofetil
Increased plasma concentrations of both drugs.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include agitation, coma, seizures, and lethargy. Acute renal failure has been reported. Management involves symptomatic and supportive treatment. Hemodialysis can significantly remove acyclovir from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but human studies are limited. Use only if clearly needed. Acyclovir is excreted into breast milk; use with caution in breastfeeding mothers.
