Description
Overview of the medicine
Acetylcysteine is a mucolytic agent that helps to thin and loosen mucus in the airways, making it easier to cough up. It is often used in conditions where excessive or thick mucus is a problem.
Uses & Indications
Dosage
Adults
Mucolytic: 200 mg 2-3 times daily. Dissolve the granules in half a glass of water and drink immediately. For paracetamol overdose, specific protocols apply, often involving higher initial doses and maintenance doses.
Elderly
No specific dosage adjustment usually required, use adult dose.
Renal_impairment
No specific dosage adjustment needed for mucolytic use.
How to Take
Completely dissolve the contents of one sachet in half a glass of water. Drink the solution immediately after dissolution. Do not chew or swallow the granules directly.
Mechanism of Action
Acetylcysteine works by breaking the disulfide bonds within mucoproteins, which are components of mucus. This action reduces the viscosity of mucus, making it less tenacious and easier to clear from the respiratory tract.
Pharmacokinetics
Onset
Mucolytic effect usually within 1-2 hours.
Excretion
Mainly renal as inactive metabolites and inorganic sulfates.
Half life
Approximately 5.6 hours (reduced to 2 hours in severe hepatic impairment).
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentration reached within 1-2 hours.
Metabolism
Primarily hepatic, deacetylation to cysteine and further metabolism to various metabolites.
Side Effects
Contraindications
- Hypersensitivity to Acetylcysteine or any component of the formulation.
- Children under 2 years of age (for mucolytic use, especially for effervescent forms due to potential for aspiration).
- Active peptic ulceration (relative contraindication due to potential for gastric irritation).
Drug Interactions
Nitroglycerin
Concomitant administration may potentiate the vasodilating effects of nitroglycerin, leading to enhanced hypotension.
Cough suppressants
Do not combine with cough suppressants as the reduction of cough reflex could lead to accumulation of secretions.
Antibiotics (e.g., Tetracyclines, Aminoglycosides, Penicillins)
Acetylcysteine may chemically interact with certain antibiotics. To avoid this, administer Acetylcysteine at least 2 hours before or after antibiotics.
Storage
Store in a cool, dry place, below 30°C. Protect from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, diarrhea, and abdominal pain. Treatment is symptomatic and supportive. Gastric lavage may be considered in recent large ingestions.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed and the potential benefits outweigh the risks. Excretion into breast milk is unknown; use with caution in nursing mothers.

WHO Essential Medicine
YesClinical Trials
Acetylcysteine has been extensively studied in clinical trials for its mucolytic efficacy in various respiratory conditions and its role as an antidote in paracetamol poisoning. Ongoing research explores its broader antioxidant and anti-inflammatory roles.
Lab Monitoring
- In cases of paracetamol overdose, liver function tests (ALT, AST, bilirubin, prothrombin time) should be closely monitored.
- For long-term use or in specific conditions, electrolytes may be monitored.
Doctor Notes
- Advise patients to always dissolve the granules completely in water before ingestion to maximize efficacy and minimize potential for local irritation.
- Exercise caution in patients with a history of asthma due to the potential for bronchospasm, and monitor closely for any respiratory distress.
- Consider the potential for drug interactions, especially with antibiotics and nitroglycerin.
Patient Guidelines
- Ensure the granules are completely dissolved in water before drinking.
- Do not exceed the recommended dose without consulting your doctor.
- If you have asthma or a history of peptic ulcers, inform your doctor before taking this medicine.
- Stay hydrated by drinking plenty of fluids to aid mucus clearance.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Acetylcysteine is generally not expected to impair the ability to drive or operate machinery. However, if you experience side effects like headache or dizziness, avoid these activities.
Lifestyle Advice
- Avoid smoking, as it can worsen respiratory conditions and increase mucus production.
- Maintain good hydration by drinking adequate fluids.
