Vitalgin is a combination medication containing a muscle relaxant (Orphenadrine Citrate) and an analgesic (Paracetamol). It is used to relieve pain associated with acute painful musculoskeletal conditions like sprains, strains, and lower back pain.
Uses & Indications
Dosage
Adults
1-2 tablets, 2-3 times daily, or as directed by physician.
Elderly
Lower doses or increased interval may be considered due to potential for increased sensitivity to anticholinergic effects and slower metabolism.

Mechanism of Action
Paracetamol works by inhibiting prostaglandin synthesis in the central nervous system, leading to analgesic and antipyretic effects. Drug Interactions
Alcohol
Increased sedation and increased risk of Paracetamol-induced liver toxicity.
Warfarin
Chronic high doses of Paracetamol may potentiate the anticoagulant effect of warfarin.
Anticholinergics
Additive anticholinergic effects (e.g., dry mouth, blurred vision, urinary retention).
Other CNS depressants (e.g., sedatives, tranquilizers)
Additive CNS depressant effects leading to increased drowsiness.

Overdose
Paracetamol overdose can cause hepatic necrosis (symptoms: nausea, vomiting, abdominal pain, jaundice). Antidote: N-acetylcysteine. Orphenadrine overdose symptoms include anticholinergic effects (dry mouth, blurred vision, tachycardia, urinary retention, hallucinations, seizures, coma). Treatment: Symptomatic and supportive, physostigmine might be used for severe anticholinergic symptoms.
Pregnancy & Lactation
Pregnancy Category B/C for Orphenadrine (caution advised). Paracetamol is generally considered safe in pregnancy at therapeutic doses. This combination should be used during pregnancy or lactation only if clearly needed and the potential benefits outweigh the risks. Orphenadrine may be excreted in breast milk; avoid during lactation if possible.
