Description
Overview of the medicine
Viton-B 5 mg Tablet contains Pyridoxine Hydrochloride, also known as Vitamin B6. It is essential for various metabolic processes, including protein, fat, and carbohydrate metabolism, and the formation of red blood cells and neurotransmitters. It is used to prevent and treat Vitamin B6 deficiency.
Uses & Indications
Dosage
Adults
For Vitamin B6 deficiency: 2.5-10 mg daily for 3 weeks, then 2-5 mg daily as maintenance. For isoniazid-induced neuropathy: 10 mg daily. For severe deficiency: 50 mg daily up to 200 mg daily.
Elderly
No specific dose adjustment required, generally same as adults, but monitor for adverse effects.
Renal_impairment
Use with caution in severe renal impairment, dose adjustment may be necessary for higher doses, but generally safe for 5 mg.
How to Take
Administer orally, with or without food. Can be taken as a single daily dose.
Mechanism of Action
Pyridoxine (Vitamin B6) is a coenzyme involved in over 100 enzyme reactions, primarily in amino acid metabolism. It plays a crucial role in the synthesis of neurotransmitters (e.g., serotonin, dopamine), hemoglobin, and in gluconeogenesis. It is converted to its active forms, pyridoxal phosphate (PLP) and pyridoxamine phosphate (PMP), which are cofactors for many enzymes.
Pharmacokinetics
Onset
Effects typically seen within days to weeks for deficiency states.
Excretion
Excreted mainly in the urine as 4-pyridoxic acid and other inactive metabolites.
Half life
Approximately 15-20 days (for pyridoxal 5-phosphate), but elimination half-life of pyridoxine is short (2-3 hours).
Absorption
Readily absorbed from the gastrointestinal tract, primarily in the jejunum. Bioavailability is high.
Metabolism
Metabolized in the liver to its active forms, pyridoxal phosphate (PLP) and pyridoxamine phosphate (PMP), and other metabolites. Primary active form is PLP.
Side Effects
Contraindications
- Hypersensitivity to pyridoxine or any component of the formulation.
- Patients receiving levodopa monotherapy (can reduce levodopa efficacy, unless combined with carbidopa).
Drug Interactions
Levodopa
Pyridoxine can reverse the therapeutic effect of levodopa, especially in patients taking levodopa alone without a dopa decarboxylase inhibitor (like carbidopa).
Isoniazid
Isoniazid can cause pyridoxine deficiency by forming an inactive hydrazone with pyridoxine, necessitating pyridoxine supplementation.
Oral Contraceptives
May increase the requirement for pyridoxine.
Phenobarbital, Phenytoin
Pyridoxine may decrease serum concentrations of these anticonvulsants.
Storage
Store below 30°C in a dry place, protected from light and moisture.
Overdose
Acute overdose is rare and generally not severe. Chronic high doses (over 200 mg/day for prolonged periods) can lead to sensory neuropathy, ataxia, and skin lesions. Treatment involves discontinuation of the supplement.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses for deficiency. High doses should be avoided unless medically indicated.
