Overview of the medicine
Voltid-SR 100 mg Capsule contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain, inflammation, and stiffness caused by various conditions such as arthritis, ankylosing spondylitis, and acute musculoskeletal disorders. The sustained-release formulation provides prolonged therapeutic effect.
Uses & Indications
Dosage
Adults
Usually 100 mg once daily. It should be taken preferably with food.
Elderly
The lowest effective dose should be used for the shortest possible duration. Caution is advised due to increased risk of adverse effects.
Renal_impairment
Use with caution. Dose reduction or avoidance may be necessary in severe renal impairment. Regular monitoring of renal function is recommended.
How to Take
Voltid-SR 100 mg capsules should be swallowed whole with a glass of water, preferably with or after food. Do not chew, crush, or break the capsule, as this may disrupt the sustained-release mechanism.
Mechanism of Action
Diclofenac inhibits the cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By inhibiting their synthesis, diclofenac reduces these symptoms.

Onset
Onset of pain relief is typically within 1-2 hours for the SR formulation.
Excretion
Approximately 60% of the dose is excreted in the urine as metabolites, and about 35% is eliminated via bile and feces.
Half life
The plasma elimination half-life of diclofenac is approximately 1-2 hours, but the SR formulation allows for once-daily dosing due to extended release.
Absorption
Well absorbed from the gastrointestinal (GI) tract after oral administration. The sustained-release (SR) formulation provides slower and more prolonged absorption compared to immediate-release forms.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation followed by glucuronidation.

Storage

Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is primarily supportive; gastric lavage and activated charcoal may be considered within an hour of ingestion. Monitor vital signs and provide symptomatic treatment as needed.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to potential risks to the fetus and complications during delivery. In the first and second trimesters, use only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Diclofenac is excreted in breast milk; use with caution during lactation, only if deemed essential by a physician.
