Vomiren 5 mg Capsule contains Domperidone, an antiemetic and prokinetic agent used to treat nausea, vomiting, and certain gastrointestinal motility disorders. It helps to move food through your stomach more quickly and reduces feelings of sickness.
Uses & Indications
Dosage
Adults
Typically 5-10 mg (1-2 capsules) three times daily, 15-30 minutes before meals. The maximum recommended duration of treatment should not usually exceed one week. Maximum daily dose is 30 mg.
Elderly
Use the lowest effective dose for the shortest possible duration. Careful monitoring is advised due to potential for increased cardiac risk.
Renal_impairment
In severe renal impairment (creatinine clearance < 30 mL/min), the dosing frequency should be reduced to once or twice daily, depending on the severity, and dose reduction may be considered.
How to Take
Take Vomiren 5 mg Capsule orally, 15-30 minutes before meals. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Domperidone acts as a peripheral dopamine D2-receptor antagonist. It blocks dopamine receptors in the chemoreceptor trigger zone (CTZ) located outside the blood-brain barrier, thereby preventing nausea and vomiting. It also enhances gastrointestinal motility by increasing lower esophageal sphincter pressure and accelerating gastric emptying without affecting gastric secretion.

Excretion
Excreted primarily via feces (approximately 66%) and urine (approximately 33%), mostly as metabolites. Only a small amount is excreted unchanged.
Half life
Approximately 7-9 hours in healthy subjects, prolonged in patients with severe renal impairment.
Absorption
Rapidly absorbed from the gastrointestinal tract; however, oral bioavailability is low (approximately 15%) due to extensive first-pass metabolism in the liver and gut wall.
Metabolism
Extensively metabolized in the liver by N-dealkylation and oxidative hydroxylation, primarily via CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to domperidone or any component of the capsule
- Patients with moderate to severe hepatic impairment
- Patients with known prolongation of cardiac QTc intervals, significant electrolyte disturbances, or underlying cardiac diseases (e.g., congestive heart failure)
- Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
- Patients with gastrointestinal hemorrhage, mechanical obstruction, or perforation
Drug Interactions
Anticholinergics
May antagonize the prokinetic effect of domperidone.
Dopamine agonists (e.g., bromocriptine, cabergoline)
Domperidone may antagonize their peripheral effects. Domperidone can increase prolactin levels, which can interfere with dopamine agonist therapy.
Strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
Increased plasma levels of domperidone, increasing the risk of QTc prolongation. Concomitant use is contraindicated.

Overdose
Symptoms of overdose may include drowsiness, disorientation, extrapyramidal reactions (e.g., dystonia, akathisia), and cardiac arrhythmias. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if appropriate. ECG monitoring is recommended.
Pregnancy & Lactation
Pregnancy: Limited data on domperidone use in pregnant women. Should be avoided unless the potential benefits outweigh the potential risks to the fetus. Lactation: Domperidone is excreted in small amounts in breast milk. A risk to the breast-fed infant cannot be excluded, particularly in preterm infants or infants with impaired renal function.
