Overview of the medicine
Ondansetron is used to prevent nausea and vomiting caused by chemotherapy, radiation therapy, and surgery.
Uses & Indications
Dosage
Adults
For chemotherapy-induced nausea and vomiting: Typically 8 mg orally 30 minutes before chemotherapy, followed by 8 mg 8-12 hours later. For PONV: 8 mg orally one hour before anesthesia or 8 mg orally after surgery.
Elderly
No specific dose adjustment is generally required for elderly patients based on age alone, but liver function should be monitored.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take Vomiset-8 mg tablet orally, with or without food, as directed by your doctor. Swallow the tablet whole with water.
Mechanism of Action
Ondansetron selectively blocks serotonin 5-HT3 receptors located both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone, thereby preventing nausea and vomiting.
Pharmacokinetics
Onset
Approximately 30 minutes following oral administration.
Excretion
Approximately 5-10% excreted unchanged in urine. The remainder is excreted as metabolites in urine and feces.
Half life
Approximately 3 to 5 hours in adults, longer in elderly patients and those with severe hepatic impairment.
Absorption
Rapid and complete absorption from the gastrointestinal tract, with bioavailability of approximately 60% due to first-pass metabolism.
Metabolism
Extensive hepatic metabolism, primarily via hydroxylation followed by glucuronide or sulfate conjugation. Involves CYP1A2, CYP2D6, and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to ondansetron or any component of the formulation.
- Concomitant use with apomorphine due to the risk of profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
Ondansetron may reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use is contraindicated due to severe hypotension and loss of consciousness.
Phenytoin, Carbamazepine, Rifampicin
May decrease ondansetron plasma concentrations due to enzyme induction.
QT-prolonging drugs (e.g., Amiodarone, Chlorpromazine)
Increased risk of QT interval prolongation and torsades de pointes. Concurrent use should be approached with caution.

Overdose
Symptoms of overdose include transient visual disturbances, severe constipation, hypotension, and a vasovagal episode with transient AV block. Treatment should be symptomatic and supportive, as there is no specific antidote.

Pregnancy Category B. Studies in animals have not shown a risk, but well-controlled human studies are lacking. Use only if clearly needed. Ondansetron is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
