Overview of the medicine
Vonobest (Vonoprazan) is a potassium-competitive acid blocker (P-CAB) used for the treatment of acid-related disorders and as an adjunct in H. pylori eradication.
Uses & Indications
Dosage
Adults
Gastric Ulcer: 20 mg once daily for 8 weeks. Duodenal Ulcer: 20 mg once daily for 6 weeks. Reflux Esophagitis: 20 mg once daily for 4-8 weeks. H. pylori Eradication: 20 mg twice daily with antibiotics for 7 days.
Elderly
No specific dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is usually required for patients with renal impairment.
How to Take
Take orally, once daily, preferably before breakfast. For H. pylori eradication, take twice daily with concomitant antibiotics as directed by the physician.
Mechanism of Action
Vonoprazan reversibly inhibits H+, K+-ATPase (proton pump) by competing with potassium ions at the luminal surface of the gastric parietal cells, leading to potent and sustained suppression of gastric acid secretion.
Pharmacokinetics
Onset
Rapid onset of acid suppression, typically within 1 hour.
Excretion
Approximately 60% excreted renally and 30% fecally.
Half life
Elimination half-life is approximately 7-9 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached within 1.5-2 hours.
Metabolism
Primarily metabolized in the liver by CYP3A4, with minor contributions from CYP2B6, CYP2D6, and CYP2C19.
Side Effects
Contraindications
- Known hypersensitivity to vonoprazan or any component of the formulation.
- Concomitant use with atazanavir or nelfinavir due to significant reduction in antiviral efficacy.
Drug Interactions
Clopidogrel
While less severe than with PPIs, potential for interaction exists. Monitor patient response.
Atazanavir, Nelfinavir
Contraindicated. Vonoprazan significantly decreases plasma concentrations of these antivirals.
Drugs dependent on gastric pH for absorption (e.g., Ketoconazole, Itraconazole, Iron salts)
Vonoprazan may alter the absorption of these drugs due to increased gastric pH. Monitor and adjust dosage if necessary.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is unknown if vonoprazan is excreted in human milk.
