Overview of the medicine
Vonoprazan is a potassium-competitive acid blocker (P-CAB) used for the treatment of acid-related disorders. It effectively suppresses gastric acid secretion, providing rapid and sustained relief from symptoms and aiding in ulcer healing.
Uses & Indications
Dosage
Adults
For gastric ulcer: 20 mg once daily for 8 weeks. For duodenal ulcer: 20 mg once daily for 6 weeks. For reflux esophagitis: 20 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily for 7 days in combination with antibiotics.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take the tablet orally, with or without food. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Vonoprazan reversibly and competitively inhibits H+/K+-ATPase (the proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion. Its binding is potassium-competitive, leading to potent and prolonged acid suppression independent of proton pump activation.

Onset
Acid suppression begins within 1-2 hours.
Excretion
Excreted renally (approx. 50%) and fecally (approx. 35%).
Half life
Approximately 7-9 hours, longer than most PPIs.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations within 1.5-2 hours.
Metabolism
Primarily metabolized by cytochrome P450 enzymes, mainly CYP3A4, and to a lesser extent by CYP2B6, CYP2C19, and CYP2D6.

- Hypersensitivity to vonoprazan or any component of the formulation.
- Coadministration with atazanavir or nelfinavir due to potential significant reduction in their plasma concentrations.
Drug Interactions
Clopidogrel
Vonoprazan may reduce the antiplatelet effect of clopidogrel, though clinical significance is still under investigation.
Atazanavir/Nelfinavir
Coadministration is contraindicated due to decreased plasma levels of these antivirals.
pH-dependent absorption drugs (e.g., Ketoconazole, Itraconazole)
